保阴煎加味对围绝经期功能失调性子宫出血虚热证疗效及血清血管内皮生长因子和碱性成纤维细胞生长因子水平的影响

摘　　要：目的探讨保阴煎加味对围绝经期功能失调性子宫出血(dysfunctional uterine bleeding,DUB)虚热证的疗效以及血清血管内皮生长因子(VEGF)和碱性成纤维细胞生长因子(bFGF)水平的影响。方法纳入2018年4月-2021年4月在德清县中医院治疗的DUB患者68例,随机分成对照组和治疗组各34例。对照组予黄体酮胶囊;治疗组于对照组的基础上予保阴煎加味内服;两组均观察3个月经周期。比较两组完全止血时间、虚热证症状评分、子宫相关指标改善、性激素水平、临床疗效以及血清VEGF和bFGF水平。结果治疗组的完全止血时间显著短于对照组(P<0.01);治疗后,治疗组的虚热证症状评分明显低于对照组(P<0.01);治疗后,两组子宫内膜厚度及血清促卵泡生成素(FSH)、黄体生成素(LH)、雌激素(E2)水平显著减少,子宫平滑肌收缩幅度明显增加,且治疗组以上指标改善均明显对照组(P<0.01);治疗组的总有效率明显高于对照组(P<0.05);治疗后,两组血清VEGF和bFGF水平明显增加,且治疗组明显高于对照组(P<0.01)。结论在西医常规治疗基础上,保阴煎加味能更有效地控制DUB虚热证患者的出血症状,促进中医证候、性激素水平改善,提高治疗效果,上调VEGF和bFGF水平。Objective To investigate the effect of Baoyin Decoction(保阴煎)on perimenopausal dysfunctional uterine bleeding(DUB)with deficiency heat syndrome and its effect on serum levels of vascular endothelial growth factor(VEGF)and basic fibroblast growth factor(bFGF).Methods Sixty-eight DUB patients treated at Deqing Hospital of Traditional Chinese Medicine from April of 2018 to April of 2021 were enrolled and randomly divided into control group and treatment group with 34 patients in each group.The control group was given progesterone capsules,and on the basis of the control group,the treatment group was given Baoyin Decoction orally.Two groups were observed for 3 menstrual cycles.The time of complete hemostasis,the symptom score of deficiency heat syndrome,the improvement of uterus related indicators,the sex hormone levels,the clinical efficacy and the serum levels of VEGF and bFGF were compared between the two groups.Results The complete hemostasis time in the treatment group was significantly shorter than that in the control group(P<0.01).After treatment,the symptom scores of deficiency heat syndrome in the treatment group were significantly lower than those in the control group(P<0.01).After treatment,the endometrial thickness and the serum levels of follicle-stimulating hormone(FSH),luteinizing hormone(LH)and estrogen(E2)were significantly decreased.The uterine smooth muscle contraction was significantly increased in both groups,and the above indexes in the treatment group were significantly improved than those in the control group(P<0.01).The total effective rate in the treatment group was significantly higher than that in the control group(P<0.05).After treatment,the serum levels of VEGF and bFGF in the two groups were significantly increased,and the levels in the treatment group were significantly higher than those in the control group(P<0.01).Conclusion Based on the conventional treatment of Western medicine,Baoyin Decoction can more effectively control the bleeding symptoms of DUB patients with deficiency
