摘 要:目的探讨P13K/AKT-miR-126通路在儿童支气管哮喘缓解期的作用与中医证型的关系。方法选取2020年1月—2021年12月深圳市儿童医院收治的80例哮喘缓解期儿童作为哮喘组,再进行中医辨证。选入同期30例体检健康儿童作为对照组。测定各组外周血单核细胞miR-126、PI3K、AKT1表达,和血清炎症因子水平、肺功能,并进行统计分析。结果患儿组miR-126、PI3K、AKT1、IL-6、IL-1表达均高于对照组(均P<0.01)。肺脾气虚型组miR-126、PI3K、AKT1、IL-6、IL-1表达均高于肺肾两虚型组;而FVC%、FEV1%均低于肺肾两虚型组(均P<0.01)。儿童哮喘缓解期miR-126表达与PI3K、AKT1、IL-6、IL-1表达均呈正相关;而与FVC%、FEV1%均呈负相关;排除传统因素(性别、年龄、病程)的影响,多元线性回归分析进一步显示miR-126表达与FVC%、FEV1%仍呈负相关(均P<0.01)。外周血单核细胞中miR-126>1.37、PI3K>2.02、AKT1>2.96为肺脾气虚型组发病的危险因素;miR-126>1.06、PI3K>1.90、AKT1>2.87为肺肾两虚型组发病的危险因素(均P<0.01)。结论P13K/AKTmiR-126通路在儿童支气管哮喘缓解期发生过程中发挥重要作用,且该通路在肺脾气虚型的激活程度高于肺肾两虚型。Objective To investigate the role of P13K/AKT-miR-126 pathway in bronchial asthma remission and its relationship with TCM syndrome types in children.Methods A total of 80 asthmatic children treated in Shenzhen children's hospital from January 2020 to December 2021 were selected as asthma group,and then TCM syndrome differentiation was conducted.Thirty healthy children were selected as the control group.To measure the expression of miR-126,PI3K,AKT1 in peripheral blood mononuclear cells,serum inflammatory factor levels,and lung function in each group,and conduct statistical analysis.Results The expressions of miR-126,PI3K,AKT1,IL-6 and IL-1 in the pediatric group were higher than those in the control group(all P<0.01).The expression of miR-126,PI3K,AKT1,IL-6 and IL-1 in the group with lung-Qi deficiency were higher than those in the group with lung-kidney deficiency.FVC%and FEV1%were lower than those of the group with lung and kidney deficiency(all P<0.01).The expression of miR-126 was positively correlated with the expression of PI3K,AKT1,IL-6 and IL-1 in children with asthma in remission.It was negatively correlated with FVC%and FEV1%.Excluding the influence of traditional factors(gender,age and course of disease),multiple linear regression analysis further showed that miR-126 expression was still negatively correlated with FVC%and FEV1%(all P<0.01).MiR-126>1.37,PI3K>2.02,AKT1>2.96 in peripheral blood mononuclear cells were the risk factors of lung Qi deficiency group.MiR-126>1.06,PI3K>1.90 and AKT1>2.87 were risk factors for the patients with lung and kidney deficiency(all P<0.01).Conclusion The P13K/AKT-miR-126 pathway plays an important role in the occurrence of bronchial asthma remission in children,and the activation degree of this pathway is higher in lung-Qi deficiency than in lung-kidney deficiency.
