基于“脑肠轴”探讨针刺治疗卒中后抑郁的作用机制

摘　　要：卒中后抑郁(PSD)是脑卒中后最常见的并发症之一,PSD患者常常伴随便秘、呃逆等胃肠道症状,严重影响患者生活质量及后期功能恢复。目前西药治疗PSD不良反应较大,且会加重胃肠道症状。针刺作为非药物疗法,具有绿色安全、操作方便等优势,是治疗PSD的常用方法之一。脑肠轴是大脑与胃肠道的双向调节通道,文章通过梳理PSD的发病机制、脑肠轴对PSD的影响,从针刺调节脑肠肽、肠道菌群、下丘脑-垂体-肾上腺轴(HPA)以及中医学治疗PSD的理论依据出发,多个角度探讨针刺通过脑肠轴治疗PSD的作用机制,以期为临床治疗PSD提供新的思路与方法。Post-stroke depression(PSD)is one of the most common complications after stroke.PSD patients often have gastrointestinal symptoms such as constipation and hiccup,which seriously affect the quality of life and later functional recovery.At present,the side effects of Western medicine in treating PSD are large,and it will aggravate gastrointestinal symptoms.As a green therapy,acupuncture has the advantages of green,safe and convenient operation.It is one of the common methods for treating PSD.The brain gut axis is a two-way regulatory channel between the brain and the gastrointestinal tract.By combing the pathogenesis of PSD and the influence of the brain gut axis on PSD,this paper discusses the mechanism of acupuncture treatment of PSD through the brain gut axis from multiple perspectives based on the theoretical basis of acupuncture regulation of brain-gut peptide,intestinal flora,hypothalamus-pituitaryadrenal(HPA)axis and Chinese medicine treatment of PSD,in order to provide new ideas and methods for clinical treatment of PSD.
