仲景从脾论治痹证思考

摘　　要：痹证是困扰人类健康的重要疾病之一,也是中医药治疗的优势病种之一。张仲景的痹证理论和治疗实践,被后世奉为准绳。而通过原文,剖析仲景治疗痹证的原则方法,指导临床实践,更为重要。文章着眼于仲景原文,从《伤寒论》《金匮要略》中的痹证有关的方证中,从健脾顾内,微汗祛湿;宣表化湿,畅中健脾;利湿健脾,益卫实表;温阳散寒,宣痹止痛;温阳化湿,运脾和中;补气助阳,和营行血;温中散寒,培土治水;理脾柔肝,缓急止痛8个方面,探求医圣从脾论治痹证的思想,并进而推广仲景以脾胃为本论治杂病的理论,以期对杂病临床诊治有所裨益。Bi syndrome is an important diseases which perplex human health,and is also one of the preponderant diseases treated by traditional Chinese medicine.ZHANG Zhongjing's theory and practice of Bi syndrome had been regarded as the criterion for thousands of years.It is more important to analyze the principle and method of treating Bi syndrome and guide the clinical practice based on ZHANG Zhongjing's original text.This paper discussed the prescription and syndrome of Bi syndrome in Treatise on Febrile Diseases and Synopsis of Golden Chamber,from the aspects of invigorating the spleen to nourish the internal and eliminating dampness by slight sweating,dispersing and urinating dampness by smoothing the middle,invigorating the spleen to strengthen the surface,regulating dampness and strengthening the spleen,warming Yang and dispelling cold to relieve pain,warming spleen Yang to dissipate dampness,replenishing Qi to promote blood circulation,cultivatings soil to cure water,regulating the spleen and liver to relieve pain.Furthermore,ZHANG Zhongjing's theory of treating miscellaneous diseases based on the spleen should be popularized to benefit the clinical diagnosis and treatment of miscellaneous diseases.
