基于玄府理论探讨健脾开玄解毒汤联合中药药浴治疗斑块状银屑病临床观察

摘　　要：目的基于玄府理论探讨健脾开玄解毒汤联合中药药浴治疗斑块状银屑病临床观察。方法选择医院治疗的斑块型银屑病患者68例。根据随机数字表法随机分为对照组与治疗组各34例。对照组给予窄谱中波紫外线光疗联合甲氨蝶呤片口服治疗,治疗组在对照组基础上给予健脾开玄解毒汤联合中药药浴治疗。观察两组治疗前后的临床疗效,PASI评分、皮损面积评分、皮损严重程度(红斑、浸润、鳞屑)评分,血清白细胞介素-1家族细胞因子促炎因子(IL-1α、IL-1β、IL-18、IL-33)表达水平,斑块型银屑病病变组织K17表达水平及复发率。结果治疗组治疗后总有效率为90.91%(30/33)显著高于对照组72.73%(24/33)(P<0.05);治疗后两组皮损严重程度评分,PASI评分,皮损面积评分,血清IL-1α、IL-1β、IL-18、IL-33水平,斑块型银屑病病变组织K17表达水平均较治疗前降低,且治疗组低于对照组(P<0.05);治疗后随访3个月,治疗组复发率为3.03%(1/33)显著低于对照组18.18%(6/33)(P<0.05)。结论健脾开玄解毒汤联合中药药浴治疗斑块状银屑病临床疗效显著,复发率低。Objective To explore the clinical observation of Jianpi Kaixuan Jiedu Decoction(健脾开玄解毒汤)combined with traditional Chinese medicine medicated bath in the treatment of plaque psoriasis based on Xuanfu theory.Methods A total of 68 patients with plaque psoriasis who were admitted to the outpatient department of dermatology and hospitalized in the ward of our hospital were selected.According to the random number table method,they were randomly divided into the control group and the treatment group,34 cases in each group.The control group was given conventional Western medicine treatment,and the treatment group was given Jianpi Kaixuan Jiedu Decoction combined with traditional Chinese medicine medicated bath on the basis of the control group.The clinical efficacy of the two groups before and after treatment,PASI score,skin lesion area score,skin lesion severity score(erythema,infiltration,scale)score,serum interleukin-1 family cytokines and pro-inflammatory factors(IL-1α,IL-1β,IL-18,IL-33)expression level,K17 expression level and recurrence rate in plaque psoriasis lesions were observed.Results The total effective rate of the treatment group after treatment was 90.91%(30/33),which was significantly higher than that of the control group(72.73%,24/33,P<0.05);After treatment,two groups of skin lesion severity scores,PASI scores,skin lesion area scores,and the levels of serum IL-1α,IL-1β,IL-18 and IL-33,as well as the expression level of K17 in plaque psoriasis lesions,decreased compared with before treatment,and the treatment group was lower than the control group(P<0.05),and the treatment group was lower than the control group(P<0.05);After 3 months of follow-up after treatment,the recurrence rate in the treatment group was 3.03%(1/33),which was significantly lower than that in the control group 18.18%(6/33)(P<0.05).Conclusion Jianpi Kaixuan Jiedu Decoction combined with traditional Chinese medicine medicated bath has a significant clinical effect on plaque psoriasis.
