止嗽散加减治疗小儿社区获得性肺炎临床疗效

摘　　要：目的 探讨止嗽散加减治疗小儿社区获得性肺炎(CAP)的疗效。方法 选取2019年1月—2021年10月河南省安阳市中医院儿科收治的90例痰热闭肺证CAP患儿,采用随机数字表法分为观察组45例与对照组45例。对照组采用常规西药治疗,观察组采用止嗽散加减治疗,均连续治疗7 d。治疗后,评估两组临床疗效。比较两组治疗前后中医证候积分、C反应蛋白(CRP)、降钙素原(PCT)、凝血酶原时间(PT)、凝血酶时间(TT)和D-二聚体(DD)水平;比较两组治疗期间不良反应发生率。结果 观察组治疗有效率为91.11%(41/45),显著高于对照组的75.56%(34/45)(P<0.05);两组痰热闭肺证CAP患儿治疗前发热烦躁、气急鼻煽和口唇微绀等中医证候积分差异无统计学意义(P>0.05);治疗后,观察组各项中医证候积分均低于对照组(P<0.05);观察组TT、PT和DD均明显低于对照组(P<0.05);观察组血清CRP和PCT水平低于对照组(P<0.05);观察组不良反应发生率为13.33%(6/45),与对照组的17.78%(8/45)差异无统计学意义(P>0.05)。结论 止嗽散加减可提高CAP患儿临床疗效,改善凝血功能,调节炎性因子水平,安全性好。Objective To explore the curative effect of Modified Zhisou Powder(止嗽散加减)on community acquired pneumonia(CAP)in children.Methods A total of 90 children with CAP of phlegm-heat blocking lung syndrome admitted to Pediatrics Department of Anyang Hospital of Traditional Chinese Medicine were enrolled between January 2019 and October 2021.According to the random number table method,they were divided into the observation group and the control group,45 cases in each group.The control group was treated with the routine Western medicine,while the observation group was treated with Modified Zhisou Powder.After 1 week,the clinical curative effect in both groups was evaluated.The scores of TCM symptoms,C-reactive protein(CRP),procalcitonin(PCT),prothrombin time(PT),thrombin time(TT)and D-Dimer(DD)before and after treatment,and the incidence of adverse reactions during treatment between the two groups were compared.Results The response rate in the observation group was significantly higher than that in the control group(91.11%vs 75.56%)(P<0.05).Before treatment,the difference in scores of TCM symptoms(cough and shortness of breath,high fever in the nose,lips cyanosis)between the two groups was not statistically significant(P>0.05).After treatment,the scores of TCM symptoms in the observation group were lower than those in the control group(P<0.05).The levels of PT,TT and DD in the observation group were lower than those in the control group(P<0.05),levels of CRP and PCT were lower than those in the control group(P<0.05).The difference in the incidence of adverse reactions between the observation group and the control group was not statistically significant(13.33%vs 17.78%)(P>0.05).Conclusion Modified Zhisou Powder can improve the clinical curative effect and coagulation function in CAP children and regulate the levels of inflammatory factors,with good safety.
