摘　　要：目的比较经左、右远端桡动脉(dTRA)行选择性冠状动脉造影(CAG)手术操作效果及术后并发症等方面的差异。方法回顾性分析2022年1—6月在焦作市人民医院经dTRA入径行CAG的180例患者,根据穿刺部位的不同分为经左侧dTRA(A组,91例)和右侧dTRA(B组,89例)。比较两组造影成功率、X线曝光时间、手术时间、穿刺相关并发症以及术后舒适度等情况。结果两组手术成功率、血管并发症(局部血肿、穿孔、动静脉瘘,桡动脉痉挛和神经损伤)发生率以及舒适度比较,差异无统计学意义(P>0.05);A组的手术时间、X线曝光时间均较B组短,差异有统计学意义(P<0.05)。结论左、右dTRA入径行CAG均有较高的成功率和较少的并发症,可为CAG常规入径,在临床上值得推广。左侧dTRA入径较右侧dTRA入径手术具有操作时间短、导管控制方便、X线曝光时间少等特点,具有较好的临床应用前景。Objective To compare effects and complications of selective coronary angiography(CAG)operation through left and distal transradial approach(dTRA).Methods A total of 180 patients with CAG enrolled by dTRA in Jiaozuo People’s Hospital of Henan Province from January to June 2022 were selected,and were randomly divided into 91 patients receiving left distal radioul dTRA(group A)and 89 patients receiving right distal radioul dTRA(group B).The success rate of angiography,operation time,X-ray exposure time,puncture related complications and postoperative comfort were compared between the two groups.Results There were no significant differences in the success rate of surgery and the incidence of vascular complications(local hematoma,perforation,arteriovenous fistula,radial artery spasm and nerve injury)between the two groups(P>0.05).However,the operation time and X-ray exposure time of group A were shorter than those of group B,and the difference was statistically significant(P<0.05).Conclusion Both left and right dTRA have higher success rate and fewer complications in CAG,which can be the conventional approach path of CAG and is worthy of extensive promotion in clinical practice.Compared with the right dTRA entry diameter,the left dTRA entry diameter has shorter operation time,convenient catheter control and less X-ray exposure time,and it has a good prospect of clinical application.
