摘 要:目的探讨急性呼吸窘迫综合征(ARDS)新生儿血清微小RNA(miRNA)-223、miR-424表达与病情和预后的关系。方法选取121例ARDS新生儿为ARDS组,另同期选取32例健康新生儿为对照组。采用实时荧光定量PCR法检测血清miR-223、miR-424表达;行动脉血气分析,计算氧指数(OI)。根据OI进行病情评估并将ARDS新生儿分为轻度组(n=45)、中度组(n=48)、重度组(n=28);根据28 d临床结局将ARDS新生儿分为死亡组(n=22)、存活组(n=99)。采用Spearman相关法分析ARDS新生儿血清miR-223、miR-424表达与OI的相关性;用受试者工作特征曲线分析血清miR-223、miR-424对ARDS新生儿死亡的预测价值。结果ARDS组血清miR-223、miR-424表达低于对照组(P均<0.05)。ARDS新生儿血清miR-223、miR-424表达与OI呈负相关(rs分别为-0.726、-0.776,P均<0.05)。轻度组、中度组、重度组血清miR-223、miR-424表达依次降低(P均<0.05)。血清miR-223、miR-424联合预测ARDS新生儿死亡的曲线下面积(0.873)大于二者单独预测(0.787、0.786),比较差异有统计学意义(P均<0.05)。结论ARDS新生儿血清miR-223、miR-424表达降低与病情加重和预后不良有关,二者可能成为ARDS新生儿预后评估指标。Objective To investigate the relationships between serum microRNA(miRNA)-223 and miR-424 expression and the condition and prognosis in neonates with acute respiratory distress syndrome(ARDS).Methods Totally 121 neonates with ARDS were selected as the ARDS group,and another 32 healthy neonates were selected as the control group during the same period.Serum miR-223 and miR-424 expression was detected by quantitative real-time polymerase chain reaction(RT-qPCR);oxygen index(OI)was calculated using arterial blood gas analysis.The condition was assessed and grouped into the mild group(n=45),the moderate group(n=48),and the severe group(n=28).The ARDS neonates were divided into the death group(n=22)and survival group(n=83)based on 28 d clinical outcome.Spearman's correlation method was used to analyze the correlation between serum miR-223 and miR-424 expression and OI in ARDS neonates;the receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum miR-223 and miR-424 for death in ARDS neonates.Results Serum miR-223 and miR-424 expression levels were lower in the ARDS group than in the control group(all P<0.05).Serum miR-223 and miR-424 expression in ARDS neonates was negatively correlated with OI(rs=-0.726 and-0.776,respectively;both P<0.05).Serum miR-223 and miR-424 expression decreased in the mild,moderate,and severe groups in that order(all P<0.05).ROC curve analysis showed that the area under the curve of serum miR-223 and miR-424 combined in predicting the neonatal death in ARDS(0.873)was greater than that of either alone(0.787 and 0.786),with statistically significant differences(all P<0.05).Conclusion Reduced serum miR-223 and miR-424 expression in ARDS neonates is associated with exacerbation and poor prognosis,and both of them may be indicators for prognostic assessment of ARDS neonates.
