手术室综合护理在预防老年前列腺手术患者术中低温及寒战中的应用

摘　　要：目的探讨手术室综合护理在预防老年前列腺手术患者术中低温及寒战中的应用效果。方法选取天长市人民医院2020年1月至2021年8月收治的106例择期行经尿道前列腺等离子电切术(TURP)的老年良性前列腺增生(BPH)患者,按照随机数字表法分为研究组(n=53,手术室综合护理干预)和对照组(n=53,常规优质护理干预)。比较两组手术情况、术中生命体征变化及寒战发生情况、手术并发症发生率。结果研究组手术时长短于对照组(P<0.05);研究组麻醉后10 min、持续冲洗30 min、持续冲洗60 min、冲洗完毕时和术毕各时间点的口腔体温均高于对照组,持续冲洗60 min、冲洗完毕时和术毕各时间点的心率及平均动脉压均低于对照组(P<0.05);研究组患者寒战发生率及严重程度均低于对照组(P<0.05);两组手术并发症发生率比较,差异无统计学意义(P>0.05)。结论手术室综合护理能够有效降低老年TURP患者术中低温和寒战发生风险,维持稳定血流动力,减轻应激反应。Objective To explore the application of comprehensive nusing in operating room to prevent the intraoperative hypothemia and chills in elderly patients undergoing prostate surgery.Methods A total of 106 elderly patients with benign prostatic hypeplasia(BPH)who underwent elective transurethral resection of prostate(TURP)in Tianchang People's Hospital from January 2020 to August 2021 were selected and divided into study group(n=53,comprehensive nursing intervention in operating room)and control group(n=53,routine high-quality mursing intervention)according to the random number table method.The surgical conditions,changes of intraoperative vital signs,occurrence of chills and incidence rates of surgical complications were compared between the two groups.Results The surgical duration in study group were shorter than that in control group(P<0.05).The oral body temperature in study group was higher than that in control group at 10 min after anesthesia,after 30 min of continuous irrigation,after 60 min of continuous irnigation,at the end of irigation and at the end of surgery,and the heart rate and mean arterial pressure were lower than that in control group after 60 min of continuous irigation,at the end of irigation and at the end of surgery(P<0.05).The incidence ate and severity of chills in study group were lower than those in control group(P<0.05).There was no significant difference in the surgical complication rate between the two groups(P>0.05).Conclusions Comprehensive nursing in operating room can effectively reduce the risk of intaoperative hypothemia and chills in elderly patients with TURP,maintain the stable hemodynamics,and prevent the stress response.
