摘　　要：目的采用结构方程模型的信度与效度分析评价精液参数、患者资料、性激素和甲状腺功能等因素对宫内人工授精妊娠结局的影响。方法选取2018年1月至2020年12月在邢台市第三医院生殖医学中心实施宫内人工授精的207对夫妻的387个治疗周期的临床资料和实验室检查结果作为研究对象。纳入影响因素作为模型观察变量并进行编号,将具有相同性质的观察变量归为同一潜变量,构建结构方程模型并进行信度和效度分析。结果(1)精液参数、患者资料、性激素和精液优化质量的Cronbach′sα系数为0.6~0.8,通过信度检验;甲状腺功能的Cronbach′sα系数<0.6,未通过信度检验。(2)探索性因子分析,得出精液参数、患者资料、性激素和精液优化质量具有良好的结构效度。(3)验证性因子分析结果,提示模型与实际数据拟合程度符合要求。结论模型信度和效度分析显示筛选后影响因素可用于评价宫内人工授精妊娠结局,并具有一定临床应用价值。Objective To evaluate the infuence of factors such as semen parameters,patient data,sex hormones and thyroid function on the pregnancy outcome of intrauterine insemination(IUI)using structural equation model reliability and validity analysis.Methods From January 2018 to December 2020,the clinical data and laboratory test results of 387 treatment cycles of 207 couples who underwent intrauterine insemination in the Department of Reproductive Medicine of the Third Hospital of Xingai City were selected as the research objects.The influencing factors were included as the observed variables of the model and numbered,and the structural equation model was constructed and the rel iability and validity were analyzed.Results(1)The Cronbach's ax coefficient of the four variables of the men's semen quality,patient data,sex hormones and semen optimization quality was between 0.6 and 0.8,passing the reliability test.The Cronbach's ax cofficient of thyroid function was<0.6,which failed the reliability test.(2)Exploratory factor analysis found that the men's semen quality,patient data,sex hormones and semen optimization quality had good construct validity.(3)Confimatory factor analysis results,indicated that the ftting degree of the model and the actual data meets the requirements.Conclusions The reliability and validity analysis of the model shows that the influencing factors after screening can be used to ev aluate the pregnancy outcome of IUI,and have certain clinical application value.
