摘　　要：目的分析双侧子宫动脉上行支结扎联合海曼式缝合术在剖宫产术中难治性产后出血(PPH)患者中的应用价值。方法将2018年1月至2022年6月期间海口市妇幼保健院收治的120例剖宫产术中难治性PPH患者纳入研究,依据手术方法不同分为试验组(n=60)及对照组(n=60)进行回顾性分析;观察两组手术指标、术后2 h及24 h出血量、术后7 d及3个月子宫恢复情况、月经恢复时间、子宫切除率。结果两组患者手术时间并无显著差异(P>0.05),试验组术中出血量、输血量及术后2 h、4 h出血量显著少于对照组(P<0.05),试验组术后7 d、3个月子宫恢复情况明显优于对照组(P<0.05),试验组月经恢复时间及子宫切除率较对照组更低(P<0.05)。结论剖宫产术中难治性PPH采用双侧子宫动脉上行支结扎联合海曼式缝合术可有效减少产妇术中及术后的出血量,促进子宫功能恢复,且具有较高安全性。Objective To analyze the application value of bilateral superior branch of uterine artery ligation combined with Hyman suture in patients with refractory postpartum hemorrhage(PPH)during cesarean section.Methods A total of 120 patients with refractory PPH during cesarean section admitted to Haikou Maternal and Child Health Hospital from January 2018 to June 2022 were ineluded in the study and divided into experimental group(n=60)and control group(n=60)according to operation method.The surgical indexes,blood loss 2 h and 24 h afer surgery,uterine recovery at 7 d and 3 months after surgery,menstrual ecovery time and hysterectomy rate of the two groups were observed.Results There was no significant difference in operation time between the two groups(P>0.05).The intraoperative blood loss,blood transfusion and 2 h and 4 h postoperative blood loss of experimental group were significantly lower than those of control group(P<0.05).The uterine recovery of experimental group was sigifcantly better than that of control group at 7 d,3.months after operation(P<0.05).The menstrual recovery time and hysterectomy rate of experimental group were lower than those of control group(P<0.05).Conclusions For refractory PPH during cesanrean section,bilateral superior branch of uterine artery ligation combined with Hyman suture can effectively reduce intraoperative and postoperative blood loss,promote the recovery of uterine function with a high safety.
