易医脐针联合填精化瘀方治疗宫腔镜下宫腔粘连分解术后患者的临床疗效

摘　　要：目的探究易医脐针联合填精化瘀方治疗宫腔镜下宫腔粘连分解术(TCRA)后患者的临床疗效和作用机制。方法选取2021年3月至2022年9月辽宁中医药大学附属第一医院妇科门诊诊治的宫腔粘连(IUA)肾虚血瘀证患者作为研究对象。采用随机数字表法分为口服对照组(口服组,采用西药治疗,n=31)和脐针治疗组(脐针组,采用易医脐针+填精化瘀方+西药治疗,n=32)。比较两组的治疗效果。结果(1)治疗后,两组IUA评分均低于治疗前,且脐针组低于口服组,差异具有统计学意义(P<0.05)。(2)治疗后,两组子宫内膜厚度(Em)均较治疗前增加,搏动指数(PI)均较治疗前减少,仅脐针组的阻力指数(RI)较治疗前降低,且脐针组Em高于口服组,PI、RI低于口服组,差异具有统计学意义(P<0.05)。(3)治疗后,两组中医证候积分均低于治疗前,且脐针组低于口服组,差异具有统计学意义(P<0.05)。结论易医脐针联合填精化瘀方能够显著提高IUA患者TCRA术后的临床疗效,降低IUA评分,改善子宫内膜容受性,降低中医证候积分。Objective To investigate the clinical efficacy and mechanism of navel acupuncture combined with Tianjing Huayu prescription in treating patients after transcervical resection of adhesion(TCRA).Methods Intrauterine adhesion(IUA)patients with kidney deficiency and blood stasis syndrome diagnosed and treated in the gynecology clinic of the First Afiliated Hospital of Liaoning University of Ttraditional Chinese Medicine from March 2021 to September 2022 were selected as the research objects.The patients were randomly divided into two groups:oral control group(oral group,treated with western medicine,n=31)and the navel acupuncture treated group(navel acupuncture group,treated with the navel acupuncture+Tianjing Huayu prescription+westem medicine,n=32).Results(1)After treatment,the score of IUA in the two groups was lower than that before treatment,and that in the navel acupuncture group was lower than that in the oral group,and the difference was statistically significant(P<0.05).(2)After treatment,compared with before treatment,the endometrial thickness(Em)increased and the pulsatility index(PI)decreased in both groups;the resistance index(RI)in the navel acupuncture group was lower than that before treatment,and the Em in the navel acupuncture group was higher than that in the oral group,while PI and RI were lower than those in the oral group,and the dfference was satistically significant(P<0.05).(3)After treatment,the traditional Chinese medicine syndrome scores of the two groups were lower than those before treatment,and that of the navel acupuncture group was lower than that of the oral group,and the difference was staistically sigificant(P<0.05).Conclusion Navel acupuncture combined with Tianjing Huayu prescription can significantly improve the clinical effect,reduce IUA score,improve endometrial receptivity and reduce the score of traditional Chinese medicine syndrome of IUA patients after TCRA.
