针刺合谷穴、腕踝穴配合连续硬膜外药物镇痛在剖宫产镇痛中的应用研究

摘　　要：目的探讨针刺合谷穴、腕踝穴配合连续硬膜外药物镇痛在剖宫产镇痛中的作用。方法选取南京市浦口区中医院2019年5月至2022年5月收治的240例剖宫产初产妇作为研究对象。采用随机数字表法分为A组、B组、C组和对照组,各60例。在剖宫产手术麻醉中,对照组采用常规0.15%罗哌卡因,A组予以针刺合谷穴、腕踝穴配合0.08%罗哌卡因,B组予以针刺合谷穴、腕踝穴配合0.12%罗哌卡因,C组予以针刺合谷穴、腕踝穴配合0.15%罗哌卡因。比较四组麻醉效果及不良反应。结果B组、C组手术10 min、手术结束时、术后2 h疼痛评分均低于A组、对照组(P<0.05);B组、C组麻醉满意率均高于A组、对照组(P<0.05);A组、B组不良反应发生率均低于C组、对照组(P<0.05)。结论针刺合谷穴、腕踝穴配合连续硬膜外麻醉可减轻剖宫产疼痛,在降低罗哌卡因浓度的同时还可增强麻醉效果,且安全。Objective To explore the role of acupuncture at Hegu and wrist ankle points combined with continuous epidural analgesia in analgesia of cesarean section.Methods 240 primipara of cesarean section treated in Nanjing Pukou District Hospital of Traditional Chinese Medicine from May 2019 to May 2022 were.selected as research objects and divided into 4 groups with 60 cases in each by the random digital table method.During anesthesia during cesarean section,the control group was given conventional 0.15%ropivacaine,and group A was given acupuncture at Hegu point,wrist ankle point and 0.08%ropivacaine,and group B was given acupuncture at Hegu point,wrist ankle point and 0.12%ropivacaine,while group C was given acupuncture at Hegu point,wrist ankle point and 0.15%ropivacaine.The anesthetic effects and adverse reactions were compared among the four groups.Results The pain scores of group B and C at 10 min of operation,at the end of operation and after 2 h of operation were lower than those of group A and control group(P<0.05).The satisfactory rates of anesthesia in group B and C were higher than those in group A and control group(P<0.05)。The incidences of adverse reactions in group A and B were lower than those in group and control group(P<0.05).Conclusions Acupuncture at Hegu and wrist ankle points combined with continuous epidural anesthesia can reduce the pain of cesarean section,reduce the concentration of ropivacaine,and enhance the anesthetic effect at the same time,with safety.
