硬膜外阻滞联合推拿整脊治疗腰椎间盘突出症30例临床观察

摘　　要：目的:观察硬膜外阻滞联合推拿整脊治疗对腰椎间盘突出症的临床疗效及外周血细胞因子的影响,探讨其可能的作用机理。方法:随机将60例腰椎间盘突出症患者分为两组,每组30例,对照组采用硬膜外阻滞法;观察组采用硬膜外阻滞联合推拿整脊法。比较两组治疗前后的外周血细胞因子水平变化,并进行疗效评价。结果:观察组治疗后肿瘤坏死因子(TNF-α)、白细胞介素-1(IL-1)和白细胞介素-6(IL-6)下降水平显著优于对照组(P<0.05);且观察组治疗总有效率为93.3%,显著高于对照组(P<0.05)。结论:硬膜外阻滞联合推拿整脊治疗腰椎间盘突出症,有利于降低患者外周血细胞因子水平,提高临床疗效。Objective To observe the clinical efficacy of epidural block combined with massage chiropractic on lumbar intervertebral disc herniation and the effect of peripheral blood cytokines,and to explore its possible mechanism of action.Methods 60 patients with lumbar intervertebral disc herniation were randomLy divided into 2 groups,30 cases in each group.The control group was treated with epidural block,and the observation group was treated with epidural block combined with massage chiropractic.The changes of peripheral blood cytokine levels before and after treatment were compared between the two groups,and the efficacy was evaluated.Results After treatment,the levels of tumor necrosis factor(TNF-α),interleukin-1(IL-1)and interleukin-6(IL-6)in the observation group were significantly lower than those in the control group(P<0.05);And the treatment effective rate ofobservationgroup was totalled 93.3%,which was significantly higher than that in the control group(P<0.05).Conclusion Epidural block combined with massage chiropractic in the treatment of lumbar intervertebral disc herniation is beneficial to reduce the level of peripheral blood cytokines and improve clinical efficacy.
