摘 要:目的探讨不同疾病活动度系统性红斑狼疮(systemic lupus erythematosus,SLE)患者血清学指标水平变化及临床意义。方法回顾性研究临床确诊的171例SLE患者的临床资料,根据系统性红斑狼疮活动评分表(systemic lupus erythematosus disease activity index,SLEDAI)将其分为无活动期70例、轻度活动期59例、中度活动期25例和重度活动期17例,分析血清学指标与不同疾病活动度SLE间的关系。结果ANA诊断SLE的阳性率为96.5%高于抗Sm抗体56.1%和抗SSA/RO52抗体73.1%(P<0.05);抗SSA/RO52抗体诊断SLE的阳性率高于抗Sm抗体(P<0.05)。抗ds-DNA、ESR、IL-6在轻度活动期、中度活动期、重度活动期SLE间水平高于无活动期(P<0.05),与SLEDAI评分成正相关(r=0.388、0.222、0.213,P<0.05);补体C3、C4在轻度活动期、中度活动期、重度活动期SLE间水平低于无活动期(P<0.05),与SLEDAI评分呈负相关(r=-0.319、-0.310,P<0.05)。结论血清抗ds-DNA、补体C3、C4、ESR、IL-6水平可以用于SLE活动度评估,辅助SLE疾病分期。Objective To explore the changes of serological indexes in patients with systemic lupus erythematosus(SLE)at varying disease activities,and their clinical significances.Methods The clinical data of 171 SLE patients who were clinically diagnosed in our hospital were retrospectively studied.According to the systemic lupus erythematosus disease activity index(SLEDAI),they were assigned into inactive stage group(n=70),mild active stage group(n=59),moderate active stage group(n=25),and severe active stage group(n=17).The correlation between serological indexes and SLE at varying disease activities was analyzed.Results The positive rate of antinuclear antibody(ANA)to diagnose SLE was significantly higher than that of anti-Sm and anti-SSA/RO52 antibodies(96.5%vs 56.1%vs 73.1%,P<0.05),which of the anti-SSA/RO52 antibody was significantly higher than that of anti-Sm antibody(P<0.05).The levels of anti-ds-DNA,erythrocyte sedimentation rate(ESR)and interleukin-6(IL-6)in mild,moderate and severe active stage groups were significantly higher than those in inactive stage group(P<0.05),which were positively correlated with SLEDAI scores(r=0.388,0.222,and 0.213,respectively;P<0.05).The levels of complement C3 and C4 in mild,moderate and severe active stage groups were significantly lower than those in inactive stage group(P<0.05),which were negatively correlated with SLEDAI scores(r=-0.319,and-0.310,respectively;P<0.05).Conclusion Serum anti-ds-DNA,complement C3,C4,ESR,IL-6 levels can be used to assess the activity and to assist the determination of clinical staging of SLE.
