摘 要:目的探讨正压振动排痰装置对良性中心气道狭窄支架置入术后患者排痰和肺功能的作用和影响。方法接受硬质支气管镜加电子支气管镜介入治疗的良性中心气道狭窄患者32例随机分为振动正压排痰康复组(试验组)和对照组,每组16例。比较2组患者治疗后排痰、肺功能[第1秒用力呼气容积(FEV_(1))、第1秒用力呼气容积占用力肺活量比值(FEV_(1)/FVC)、呼气峰值流速(PEF)]及各项呼吸指标变化(血氧饱和度(SaO_(2))、动脉二氧化碳(PaCO_(2))、呼吸频率)。结果观察2组患者术后24 h至4周痰液颜色、镜下表现及咳痰效果,试验组排痰效果好于对照组(P<0.05)。术后2组患者肺功能FEV_(1)、FEV_(1)/FVC、PEF比较差异无统计学意义(P>0.05)。术后试验组患者呼吸频率、SaO_(2)、PaCO_(2)均优于对照组(P<0.05)。结论应用正压振动排痰装置可以改善气管镜介入治疗患者术后排痰情况,促进通气氧合改善,值得推广应用。Objective To explore the effect and influence of a vibrating positive expiratory pressure device on postoperative expectoration and lung function in patients with stent placement for tracheal stenosis.Methods A total of 32 patients with tracheal stenosis treated with rigid bronchoscope plus electronic bronchoscope interventional therapy were randomly divided into the test group and control group,with 16 cases in each group.A vibrating positive expiratory pressure device was applied to patients of test group.After treatment,postoperative expectoration,lung function indexes,including forced expiratory volume in 1 second(FEV1),the ratio of the forced expiratory volume in the first one second to the forced vital capacity of the lungs(FEV1/FVC)and peak expiratory flow(PEF),and respiration indexes,including arterial oxygen saturation(SaO_(2)),partial pressure of carbon dioxide in arterial blood(PaCO_(2))and respiratory rate were compared.Results The sputum color,microscopic appearance and expectoration effect from 24 h to 4 weeks postoperatively were significantly better in the test group than those in the control group(P<0.05).There were no significant differences in postoperative FEV1,FEV1/FVC and PEF between groups(P>0.05).Postoperative respiratory rate,SaO_(2),and PaCO_(2)in the test group were significantly better than those in the control group(P<0.05).Conclusion The application of a vibrating positive expiratory pressure device contributes to improve postoperative expectoration,ventilation and oxygenation in patients with stent placement for tracheal stenosis,which is worthy of popularization.
