温胆汤治疗对冠脉搭桥术后抑郁症氧化应激及BDNF和Ghrelin水平及睡眠的影响

摘　　要：目的 探讨温胆汤治疗对冠脉搭桥术后抑郁症患者氧化应激及脑源性神经营养因子(brain-derived neurotrophic factor, BDNF)和胃饥饿素(Ghrelin)水平及睡眠的影响。方法 选取冠脉搭桥术后抑郁症患者90例,随机分为对照组和实验组,每组45例,口服盐酸舍曲林片治疗,实验组在上述治疗干预基础上给予温胆汤治疗,治疗时间均为12周,采用汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)和抑郁自评量表(Self-rating Depression Scale, SDS)评估抑郁情况,采用匹兹堡睡眠质量指数量表(Pittsburgh Sleep Quality Index, PSQI)评估睡眠情况,检测治疗前后血清丙二醛及超氧化物歧化酶、血浆一氧化氮及内皮素-1、血清BDNF和Ghrelin水平。结果 两组治疗前的HAMD和SDS评分差异无统计学意义(P>0.05),治疗后降低(P<0.001),实验组治疗后评分下降程度明显大于对照组(P<0.001);两组治疗前的丙二醛及超氧化物歧化酶、血浆一氧化氮及内皮素-1的水平比较差异无统计学意义(P>0.05),治疗后丙二醛及内皮素-1降低(P<0.001),实验组治疗后丙二醛及内皮素-1水平下降程度明显大于对照组(P<0.001),超氧化物歧化酶及一氧化氮升高,实验组治疗后超氧化物歧化酶及一氧化氮水平升高程度明显大于对照组(P<0.001);两组治疗前的血清BDNF和Ghrelin水平比较差异无统计学意义(P>0.05),治疗后升高(P<0.001),实验组治疗后升高程度明显大于对照组(P<0.001);两组治疗前的PSQI评分比较差异无统计学意义(P>0.05),治疗后评分升高(P<0.001),实验组治疗后评分升高程度明显大于对照组(P<0.001)。结论 温胆汤治疗冠脉搭桥术后抑郁症临床效果显著,可调节患者氧化应激及BDNF和Ghrelin水平,改善抑郁症状及睡眠质量。Objective To investigate the effects of Wendan Decoction(温胆汤)on oxidative stress,levels of brain-derived neurotrophic factor(BDNF)and Ghrelin and sleep in patients with depression after coronary artery bypass grafting.Methods We collected 90 patients with depression after coronary artery bypass grafting and they were randomly divided into control group and experimental group,45 cases in each group.The patients were treated with sertraline hydrochloride tablets orally,and the experimental group was additionally treated with Wendan Decoction on the basis of the above treatment intervention.The treatment duration was 12 weeks.Hamilton Depression Scale(HAMD)and Self-rating Depression Scale(SDS)were used to assess the depression.Pittsburgh Sleep Quality Index(PSQI)was used to evaluate the sleep.The levels of serum malondialdehyde and superoxide dismutase,plasma nitric oxide,endothelin-1,BDNF and Ghrelin were detected at before and after treatment.Results There were no significant differences in HAMD and SDS scores,serum levels of malondialdehyde,superoxide dismutase,plasma nitric oxide and endothelin-1,serum levels of BDNF and Ghrelin or PSQI score between the two groups before treatment(P>0.05).HAMD,SDS and PSQI scores decreased after treatment(P<0.001),and the decrease in the experimental group was significantly greater than that in the control group(P<0.001).The levels of malondialdehyde and endothelin-1 decreased after treatment(P<0.001),and the levels of malondialdehyde and endothelin-1 and serum levels of BDNF and Ghrelin decreased significantly in the experimental group than those in the control group(P<0.001).The levels of superoxide dismutase and nitric oxide increased after treatment(P<0.001),and the levels of superoxide dismutase and nitric oxide and serum levels of BDNF and Ghrelin in the experimental group were significantly higher than those in the control group(P<0.001).Conclusion The clinical effect of Wendan Decoction on depression after coronary artery bypass grafting is remarkable.It can regul
