通督调神针法联合补阳还五汤治疗卒中偏瘫的临床疗效观察

摘　　要：目的 通督调神针法联合补阳还五汤治疗卒中偏瘫的临床疗效观察。方法 纳入80例卒中偏瘫患者,并将其随机分为观察组及对照组,每组40例。两组均给予常规治疗,对照组给予普通针刺治疗,观察组给予通督调神针法联合补阳还五汤,两组患者均治疗2周。观察两组患者治疗后的临床总有效率、美国国会卫生研究院卒中量表(National Institute of Health stroke scale, NIHSS)评分、蒙特利尔认知评估量表(Montreal Cognitive Assessmen, MoCA)、福格-米勒运动功能评分量表(Fugl-Meyer motor function assessment, FMA)评分及巴塞尔指数(Barthel index, BI)评分水平。结果 经2周治疗后,观察组总有效率为92.50%(37/40),显著优于对照组的75.00%(30/40)(P<0.05);两组NIHSS及MoCA评分均较治疗前有所改善(P<0.05),且观察组显著优于对照组(P<0.05);两组患者FMA评分均较治疗前升高(P<0.05),观察组显著优于对照组;两组的BI评分较治疗前均有显著改善(P<0.05),观察组显著优于对照组。结论 通督调神针法联合补阳还五汤治疗卒中偏瘫,可显著提高临床疗效、改善患者神经功能、运动功能及生活质量。Objective To observe the clinical efficacy of dredging Du meridian and regulating spirit acupuncture combined with Buyang Huanwu Decoction(补阳还五汤)in the treatment of stroke and hemiplegia.Methods Eighty stroke patients with hemiplegia were enrolled and randomly divided into an observation group and a control group,with 40 cases in each group.Both groups were given routine treatment.The control group was given ordinary acupuncture,and the observation group was given dredging Du meridian and regulating spirit acupuncture combined with Buyang Huanwu Decoction.Both groups were treated for 2 weeks.The total clinical response rate,National Institute of Health stroke scale(NIHSS),Montreal Cognitive Assessment(Mo-CA),Fugl-Meyer motor function assessment(FMA)and Barthel Index(BI)were observed in the two groups after treatment.Results After 2 weeks of treatment,the total effective rate in the observation group was 92.50%(37/40),which was significantly better than 75.00%(30/40)in the control group(P<0.05).The total effective rate of the observation group was significantly better than that of the control group(P<0.05).The FMA scores of the two groups were improved compared with those before treatment(P<0.05),and the score of the observation group was significantly better than that of the control group.The BI values of the two groups were higher than those before treatment(P<0.05)and the BI value of the observation group was significantly better than that of the control group.Conclusions Dredging Du meridian and regulating spirit acupuncture combined with Buyang Huanwu Decoction in the treatment of stroke hemiplegia can significantly improve the clinical efficacy,neurological function,motor function and quality of life of patients.
