摘 要:肾脏是体内高度灌注的器官,它接受了20%~25%的静息状态下心输出量的血液。流入肾脏的高血流量是调节体内平衡所必需的生理过程。即使肾脏灌注超出自身调节能力的微小变化,也会对肾功能产生显著影响。因此,肾灌注的定量可以为了解肾功能和肾(病理)生理学提供重要的信息。随着MRI技术的不断发展,动脉自旋标记(ASL)MRI技术已被广泛用于肾脏灌注评估,其可靠性和可重复性已得到认可。再加上不需要使用电离辐射,使其成为一种完全无创、普遍适用,无论年龄和肾功能损害程度,非常适合需要连续灌注监测的影像学技术。此外,ASL能够分别测量每个肾脏的灌注,以及评估肾内区域灌注变化,适用于多种疾病研究。因此,本文对ASL技术的基本概念、成像原理、可行性及可重复性的基础研究,在健康人群、肾移植、糖尿病肾病中应用最新进展进行综述,并指出目前该技术的局限性及不足之处,旨在为临床工作提供依据及思路。The kidney is a highly perfused organ in the body.It receives 20%-25%of the blood from the cardiac output at rest.The high blood flow into the kidney is a necessary physiological process to regulate homeostasis.Even minor changes in renal perfusion beyond its own regulatory capacity can have a significant impact on renal function.Therefore,the quantification of renal perfusion can provide important information for understanding renal function and renal(pathological)physiology.With the continuous development of MRI technology,arterial spin labeling(ASL)MRI technology has been widely used for renal perfusion assessment,and its reliability and repeatability have been recognized.In addition,it does not require the use of ionizing radiation,making it a completely non-invasive and universally applicable imaging technology that requires continuous perfusion monitoring regardless of age and degree of renal function damage.In addition,ASL can measure the perfusion of each kidney separately and evaluate changes in regional perfusion within the kidney,making it suitable for various disease studies.Therefore,this article reviews the basic concept,imaging principle,feasibility and repeatability of ASL technology,and the latest progress in the application of ASL technology in healthy people,kidney transplantation,and diabetes nephropathy,and points out the limitations and shortcomings of this technology at present to provide basis and ideas for clinical work.
