肺超声联合肺高分辨率CT对干燥综合征相关间质性肺病的诊断价值

摘　　要：目的观察干燥综合征相关间质性肺病患者肺超声评分及肺高分辨率CT(HRCT)评分变化,探讨肺超声联合HRCT对干燥综合征相关间质性肺病的诊断价值。方法回顾性选取2020年3月至2022年3月在九江市第一人民医院进行诊治的150例干燥综合征患者作为研究对象,根据患者是否合并相关间质性肺病分为研究组(59例,合并相关间质性肺病)及对照组(91例,未合并相关间质性肺病)。分别给予患者肺部超声及HRCT检查,分析两组患者的肺部超声评分及HRCT评分变化;采用Pearson分析肺部超声评分与HRCT评分的相关性;建立ROC曲线,分析肺部超声评分联合HRCT评分对干燥综合征相关间质性肺病的诊断价值。结果磨玻璃样影为59例干燥综合征相关间质性肺病患者最常见的影像学表现。研究组患者的肺部超声评分及HRCT评分均高于对照组,差异有统计学意义(P<0.01)。Pearson相关性分析结果显示,干燥综合征相关间质性肺病患者的肺部超声评分及HRCT评分呈正相关(r=0.531,P<0.001)。建立ROC曲线,肺部超声评分、HRCT评分、肺部超声评分联合HRCT评分的曲线下面积分别为0.792、0.911、0.950,敏感度分别为83.50%、91.20%、90.01%,特异度分别为66.10%、81.40%、88.10%,肺部超声评分联合HRCT评分对干燥综合征相关间质性肺病具有较高的诊断价值。结论干燥综合征相关间质性肺病患者的肺超声评分及HRCT评分具有良好的相关性,肺部超声评分联合HRCT评分均可作为诊断及评估干燥综合征相关间质性肺病的新型影像学方法,对患者的治疗指导及预后判定具有重要意义。Objective To observe the changes of lung ultrasound score and lung high-resolution CT(HRCT)score in patients with interstitial lung disease associated with dry syndrome,and to investigate the diagnostic value of lung ultrasound combined with HRCT on interstitial lung disease associated with dry syndrome.Methods A total of 150 patients with dry syndrome diagnosed and treated in the Jiujiang First People's Hospital from March 2020 to March 2022 were retrospectively selected as research objects,and divided into study group(59 patients with associated interstitial lung disease)and control group(91 patients with no associated interstitial lung disease)according to whether the patients had associated interstitial lung disease.Patients were given lung ultrasound and HRCT examination respectively,and the changes of lung ultrasound score and HRCT score in the two groups were analyzed.Pearson was used to analyze the correlation between lung ultrasound score and HRCT score.ROC curve was established to analyze the diagnostic value of lung ultrasound score combined with HRCT score on interstitial lung disease associated with dry syndrome.Results Ground glass shadow was the most common imaging manifestation in 59 patients with interstitial lung disease associated with dry syndrome.The lung ultrasound score and HRCT score of patients in the study group were higher than those in the control group,and the differences were statistically significant(P<0.01).Pearson correlation analysis showed that the lung ultrasound score and HRCT score of patients with interstitial lung disease associated with dry syndrome showed positive correlation(r=0.531,P<0.001).To establish the ROC curve,the area under the curve of lung ultrasound score,HRCT score and lung ultrasound score combined with HRCT score were 0.792,0.911 and 0.950,the sensitivity was 83.50%,91.20%and 90.01%,and the specificity was 66.10%,81.40%and 88.10%,respectively,and the lung ultrasound score combined with HRCT score had a high diagnostic value.Conclusion The lung ultrasound s
