摘  要:目的分析临床药师干预Ⅰ类切口手术预防性抗菌药物合理应用的效果。方法选取2020年8月至2022年8月铜陵市人民医院478例Ⅰ类切口择期手术患者的临床资料,其中2020年8月至2021年8月的230例Ⅰ类切口择期手术患者为干预前组,2021年9月至2022年8月的248例Ⅰ类切口择期手术患者为干预后组。比较临床药师干预前后抗菌药物使用情况、品种构成比、抗菌药物费用和住院总费用。结果干预后组预防性应用抗菌药物、预防性用药不合理、无指征联合用药、首次给药为术后、预防用药时间持续24~48 h、预防用药时间持续>48 h、给药剂量不合理占比低于干预前组,首次给药时间为术前0.5~2 h、预防用药时间持续<24 h占比高于干预前组,差异有统计学意义(P<0.05);干预后组广谱青霉素类、第2代头孢菌素应用率低于干预前组,第1代头孢菌素应用率高于干预前组,差异有统计学意义(P<0.05);干预后组抗菌药物费用和住院总费用低于干预前组,差异有统计学意义(P<0.05)。结论临床药师干预可提高Ⅰ类切口手术预防性抗菌药物合理应用率,保证临床用药安全。Objective To analyze the effect of clinical pharmacists'intervention in the rational use of preventive antibiotics in class Ⅰ incision surgery.Methods Clinical data of 478 patients with Class Ⅰ incision elective surgery in Tongling People's Hospital from August 2020 to August 2022 were selected as the research objects.Among them,230 patients from August 2020 to August 2021 were in the pre intervention group,and 248 patients from September 2021 to August 2022 were in the post intervention group.The use of antibacterial drugs,the proportion of varieties,the cost of antibacterial drugs and the total cost of hospitalization were compared before and after the intervention of clinical pharmacists.Results In the post intervention group,the rate of the prophylactic use of antibiotics,the prophylactic use of drugs was unreasonable,the combination of drugs without indications,the first administration of drugs was after operation,the duration of prophylactic use of drugs was 24-48 h,the duration of prophylactic use of drugs was>48 h,the dosage of drugs was unreasonable were lower than that in the pre intervention group,the rate of first administration of drugs was 0.5-2 h before operation,and the duration of prophylactic use of drugs was<24 h were higher than those in the pre intervention group,the differences were statistically significant(P<0.05).The application rate of broad-spectrum penicillin and the second generation cephalosporins in the post intervention group was lower than that in the pre intervention group,and the application rate of the first generation cephalosporins was higher than that in the pre intervention group,with statistically significant differences(P<0.05).The cost of antibiotics and the total cost of hospitalization in the post intervention group were lower than those in the pre intervention group,the differences were statistically significant(P<0.05).Conclusion The intervention of clinical pharmacists can improve the rational application rate of preventive antibiotics in class Ⅰ incision surg
