摘  要:目的了解北京丰台医院呼吸科的病原菌分布及其耐药性,为临床医师诊疗提供参考。方法回顾性分析北京丰台医院2019年1月至2022年6月呼吸科送检的病原菌标本,这些标本的鉴定和体外药敏试验结果用VITEK 2 Compact全自动微生物鉴定系统(法国生物梅里埃公司)来完成,然后利用WHONET5.6软件进行抗菌药物的耐药性分析。结果从5596份样品中分离到1287株细菌,其中以革兰氏阴性菌为主,占73.3%;标本种类包括痰标本(62.8%)、血标本(17.2%)和尿标本(12.8%)等;病原菌分布包括铜绿假单胞菌(28.0%)、肺炎克雷伯菌(22.0%)、鲍曼不动杆菌(11.5%);铜绿假单胞菌对多黏菌素、阿米卡星耐药率<10%,肺炎克雷伯菌对替加环素、多黏菌素、阿米卡星的耐药率都<20%;鲍曼不动杆菌对多黏菌素、替加环素耐药率<5%。结论北京丰台医院呼吸科的病原菌感染以革兰氏阴性菌为主,铜绿假单胞菌感染比例最高,鲍曼不动杆菌耐药率最高且对多种抗生素耐药。Objective To understand the distribution of pathogenic bacteria and their drug resistance analysis in the Respiratory Department of Beijing Fengtai Hospital,so as to provide reference for clinicians in diagnosis and treatment.Methods Retrospective analysis of pathogenic bacteria samples submitted by the respiratory department of Beijing Fengtai Hospital from January 2019 to June 2022,the identification and in vitro drug sensitivity test of these samples were completed by VITEK 2 Compact full-automatic microbial identification system(France BioMerier Company),and then the drug resistance analysis of antibacterial drugs was conducted by WHONET 5.6 software.Results There were 1287 strains of bacteria were isolated from 5596 samples,of which most were gram-negative bacteria(73.3%).The specimens included sputum(62.8%),blood(17.2%)and urine(12.8%).The pathogens were Pseudomonas aeruginosa(28.0%),Klebsiella pneumoniae(22.0%)and Acinetobacter baumannii(11.5%).Pseudomonas aeruginosa was less than 10%resistant to Polymyxin and Amikacin,and Klebsiella pneumoniae was less than 20%resistant to Tegacyclin,Polymyxin and Amikacin.The resistance rate of Acinetobacter baumannii to Polymyxin and Tegacyclin was less than 5%.Conclusion The pathogens in the Respiratory Department of Beijing Fengtai Hospital are mainly gram-negative bacteria,with the highest proportion of Pseudomonas aeruginosa infection,Acinetobacter baumannii has the highest drug resistance rate and is resistant to many antibiotics.
