摘 要:目的探讨贵州省30~79岁苗族人群尿锰水平与糖尿病前期及糖尿病的关联性。方法采用多阶段、分层整群抽样方法,排除未收集尿液者后,对贵州省30~79岁3722名苗族人群进行问卷调查、体格检查。利用电感耦合等离子体质谱法检测苗族人群尿液中锰的浓度,采用二元logistic回归分析尿锰水平与糖尿病前期及糖尿病患病风险之间的关联。结果贵州省30~79岁苗族人群糖尿病前期检出率为38.47%,糖尿病患病率为9.38%。Logistic回归分析结果显示:与第一分位(Q1)相比,尿锰浓度第三分位Q3(OR=1.363,95%CI:1.123~1.656)和第四分位Q4(OR=1.417,95%CI:1.167~1.720)与糖尿病前期患病风险呈正相关,未发现尿锰水平与糖尿病患病风险之间存在关联。亚组分析显示,女性(OR=1.792,95%CI:1.341~2.239)、患高血压(OR=2.132,95%CI:1.363~3.334)人群尿锰浓度越高,糖尿病前期风险越大。结论贵州省30~79岁苗族人群尿锰浓度升高与糖尿病前期患病风险增加有关,未发现尿锰浓度与糖尿病患病风险存在关联性。Objective To investigate the relationship between urinary manganese level and prediabetes and diabetes in Miao ethnic people from 30 to 79 in Guizhou Province.Methods A multi-stage,stratified cluster sampling method was used to conduct a questionnaire survey and physical examination among 3722 Miao ethnic people aged 30 to 79 in Guizhou Province after excluding those who did not collect urine.Inductively coupled plasma mass spectrometry(ICP-MS)was used to detect urinary manganese concentrations of Miao people,and binary Logistic regression was used to analyze the association between urinary manganese levels and the risk of prediabetes and diabetes.Results The prevalence of prediabetes and diabetes were 38.47%and 9.38%respectively in Miao people aged from 30 to 79 in Guizhou Province.Logistic regression analysis showed that compared with the first quartile(Q1),the urinary manganese concentration at the third quartile Q3(OR=1.363,95%CI:1.123-1.656)and the fourth quartile Q4(OR=1.417,95%CI:1.167-1.720)were positively correlated with the risk of prediabetes,and no correlation was found between urinary manganese level and the risk of diabetes.Subgroup analysis showed that higher urinary manganese levels were associated with an increased risk of prediabetes in women(OR=1.792,95%CI:1.341-2.239)and patients with hypertension(OR=2.132,95%CI:1.363-3.334).Conclusion Increased urinary manganese concentration is associated with an increased risk of prediabetes in Miao people aged 30-79 in Guizhou Province,and no correlation is found between urinary manganese concentration and risk of diabetes.
