度普利尤单抗治疗难治性大疱性类天疱疮1例

摘　　要：患者男,87岁。周身红斑、水疱伴瘙痒2个月。既往高血压病史20年,1年前因坠床腰椎骨折截瘫,大小便失禁,留置导尿中。临床表现及病理符合大疱性类天疱疮。治疗方案:首次度普利尤单抗600 mg皮下注射,以后300 mg每两周1次皮下注射。2周皮疹基本消退,3个月后BP180及BP230降至正常范围停药,随访至6个月未复发,且无不良反应。目前患者仍在随访中。A 87-year-old male presented with erythema,blisters and itching for 2 months.He had a history of hypertension for 20 years.One year ago,he fell off bed due to lumbar fracture,paraplegia,incontinence and indwelling in catheterization.The clinical feature and pathology were consistent with bullous pemphigoid.He was treated with duplizumab,600 mg for the first time,and then 300 mg every two weeks.Rash improved significantly after 2 week.BP180 and BP230 decreased to the normal range and stoped duplizumab after 3 months.After 6 months of follow-up,no recurrence and no adverse reactions appeared.At present,the patient is still under follow-up.
