摘　　要：目的总结国内外儿童肝移植受者医疗过渡的研究进展。方法回顾近年来国内外儿童肝移植受者医疗过渡相关研究,从概念、影响因素、过渡措施等方面进行综述。结果医疗过渡指从儿童医疗保健系统过渡到成人医疗保健系统的过程,医疗保健提供者、患者、照护者及其他政策和经济因素均会影响医疗过渡,个性化的过渡方案有助于改善患者体验和健康结局。结论建立以问题和需求为导向的过渡方案,早期开展医疗过渡,提高患者自我管理能力,积极进行信息化建设,开展多学科合作,从而改善患者健康结局。Objective To summarize the research progress of health care transition(HCT)for pediatric liver transplant recipients.Method The literatures of HCT for pediatric liver transplant recipients were reviewed,and the concept,related factors,interventions and methods of health care transition were summarized.Results HCT is the process of moving from a child/family-centered model of care to an adult or patient-centered model of health care,and influenced by health care provider,child and caregivers,and other factors such as medical policy and economic level.Personalized transition program has more benefits for improving the experience and health outcomes of patients.Conclusion problem-oriented and demand-oriented transition program is recommended,early intervention to improve self-management abilities of children,information construction of pediatric medical system and multidisciplinary team building are important for improving health outcomes of patients.
