基于数据包络分析的我国县级妇幼保健院效率评价

摘　　要：目的对我国28个省(自治区、直辖市)县级妇幼保健院效率及效率变化情况进行分析,为提高各地妇幼保健院运行效率提供理论依据。方法运用数据包络分析方法,选取CCR和BCC模型分析2019年我国28个省(自治区、直辖市)县级妇幼保健院效率,通过Malmquist指数模型评价2015—2019年妇幼保健院效率的变化情况。结果28个省(自治区、直辖市)中3个地区的DEA有效,5个地区DEA弱有效,总体而言,县妇幼保健院的产出和投入的均衡性较弱。8个地区的单纯技术效率为1,各地存在投入冗余和产出不足现象,24个地区的规模状态未达到最优,整体以规模递增为主。结论我国县级妇幼保健院运行效率整体偏低,存在投入冗余和产出不足情况,且规模效率和技术效率偏低,应根据各地实际情况,合理设置妇幼保健院的规模,适当控制机构数量、人员数量和床位数量,注重县级妇幼保健院的技术服务水平和技术人才的培养,处理好规模扩大与质量改进的关系。Objective To analyze theefficiency and changes in maternity and child healthcare hospitalsat the county levelin 28 provinces(autonomous regions and municipalities directly under the Central Government)of China,so asto provide a theoretical basis for improving the operating eficiency of these hospitals.MethodsWith data envelopment analysis method the CCR and BCC models were used to analyze the operatingefficiency of maternity and child healthcare hospitals at the county level in 28 provinces(autonomous regions and municipalitiesdirectly under the Central Government)of China in 2019.The Malmquist index model was used to evaluate the changes in the efficiency of these hospitals from 2015 to 2019.Results DEA in 3 regions of the 28 provinces(autonomous regions and municipalities directly under the Central Government)was effective,and DEA in 5 regions was weak.Overall,the balance between output and input of maternity and child healthcare hospitals at the county level wasweak.The pure technical efficiency of 8 regions was 1,and there were redundant input and insufficient output in the regions.The scale of 24 regions did not reach the optimal state,but the overall scale was increasing.Conclusions The overall operating efficiency of maternity and child healthcare hospitals at the county level in China is low,with the problem of redundant input and insufficient output,as well as low scale efficiency and technical efficiency.It is vital to set up reasonably the scale of maternity and child healthcare hospitals according to the actual situation of different regions,control appropriately the number of institutions,personnel and beds,attach importance to the technical service capacity and technical staff training of maternity and child healthcare hospitals at the county level,and handle properly the relationship between scale expansion and quality improvement.
