口服钆贝葡胺在MR胰胆管成像中的应用探讨

摘　　要：目的探讨口服钆贝葡胺(Gd-BOPTA)溶液提高磁共振胰胆管成像(MRCP)图像质量的最低浓度。方法体外实验:将0.05%、0.1%、0.2%、0.3%、0.4%的Gd-BOPTA溶液及温开水分别放入50 mL注射器中,进行MRCP扫描,获取各组Gd-BOPTA溶液的信号强度及信号衰减率。临床实验:依据体外实验结果,将102例被检者随机分为3组,分别口服0.1%、0.2%、0.3%的Gd-BOPTA溶液200 mL,进行MRCP扫描,评价图像质量。结果体外实验:各组Gd-BOPTA溶液信号强度随浓度增大逐渐降低,当浓度达到0.2%时呈低信号。临床实验:0.1%Gd-BOPTA溶液组与0.2%Gd-BOPTA溶液组比较MRCP图像质量有显著性差异(P<0.05),0.2%Gd-BOPTA溶液组与0.3%Gd-BOPTA溶液组比较MRCP图像质量无显著性差异(P>0.05)。结论口服Gd-BOPTA溶液能明显提高MRCP图像质量,0.2%是进行MRCP检查时Gd-BOPTA溶液的最低浓度。Objective To investigate the minimum concentration of oral Gd-BOPTA solution to improve the image quality of magnetic resonance cholangiopancreatography(MRCP).Methods In vitro experiments,0.05%,0.1%,0.2%,0.3%,0.4%Gd-BOPTA solution and warm water were put into 50 mL syringe respectively for MRCP scanning to obtain the signal intensity and signal attenuation rate of Gd-BOPTA solution in each group.In clinical experiments,based on the results of in vitro experiments,102 subjects were randomly divided into three groups and 200 mL of 0.1%,0.2%,0.3%Gd-BOPTA solution were taken orally respectively,and MRCP scanning was performed to evaluate the image quality.Results In vitro experiments,the signal intensity of Gd-BOPTA solution in each group were decreased gradually with the increasing of concentration,and a low signal was showed when the Gd-BOPTA solution concentration was 0.2%.In clinical experiments,there was significant difference in the image quality of MRCP between the 0.1%and 0.2%Gd-BOPTA solution groups(P<0.05),and no significant difference in the image quality of MRCP between the 0.2%and 0.3%Gd-BOPTA solution groups(P>0.05).Conclusion The oral Gd-BOPTA solution can obviously improve the image quality of MRCP,0.2%is the minimum concentration of Gd-BOPTA solution for MRCP.
