外周血指标在特应性皮炎患儿中的水平变化及其临床意义

摘　　要：目的:探究特应性皮炎(atopic dermatitis,AD)患儿外周血指标中白细胞介素-13(interleukin-13,IL-13)、γ干扰素(interferon-γ,IFN-γ)、免疫球蛋白E(immunoglobulin E,IgE)及载脂蛋白A1(apolipoprotein A1,apo A1)/载脂蛋白B(apolipoprotein B,apo B)/载脂蛋白E(apolipoprotein E,apo E)水平变化及其临床意义。方法:选取2020年5月至2022年4月在青岛市市立医院接受治疗的AD患儿73例(AD组),同时选取60例健康儿童作为对照(健康组)。比较2组儿童外周血指标表达差异,同时比较不同性别、年龄、疾病严重程度AD患儿的血浆指标水平。结果:与健康组相比,AD组患儿血清IL-13、IgE、apo A1水平较高,血清apo E水平较低,差异均具有统计学意义(均P<0.05)。不同性别、不同年龄(>7岁,≤7岁)分别比较,患儿血清IL-13、IFN-γ、IgE及apo A1/B/E水平差异均无统计学意义(均P>0.05)。AD患儿血清IL-13、IgE、apo A1水平随病情加重而逐渐升高,差异均有统计学意义(均P<0.05),而轻、中、重度AD患儿的血清IFN-γ、apo B、apo E水平差异均无统计学意义(均P>0.05)。结论:血清IL-13、IgE、apo A1可能参与AD疾病进展。Objective:To investigate the changes and clinical significance in peripheral blood index of interleukin-13(IL-13),interferon-γ(IFN-γ),immunoglobulin E(IgE),and apolipoprotein A1(apo A1)/apolipoprotein B(apo B)/apolipoprotein E(apo E)in children with atopic dermatitis(AD).Methods:A total of 73 children with AD who were treated in Qingdao Municipal Hospital between May 2020 and April 2022 were selected as a AD group,and 60 healthy children were selected as a healthy group.The differences of peripheral blood index expression between the 2 groups were compared,and the plasma index levels of AD children with different gender,age,and severity were compared.Results:Compared with the healthy group,serum IL-13,IgE,and apo A1 in AD group were higher,and serum apo E was lower,and the difference was statistically significant(all P<0.05).There was no significant difference in the levels of serum IL-13,IFN-γ,IgE,and apo A1/B/E between different sexes and different ages(>7 years,≤7 years)(all P>0.05).Serum levels of IL-13,IgE,and apo A1 in children with AD increased with the severity of the disease,and the difference was statistically significant(all P<0.05).The serum levels of IFN-γ,apo B,and apo E in children with mild,moderate,and severe AD showed no statistical significance(all P>0.05).Conclusion:Serum IL-13,IgE,and apo A1 may be involved in the progression of AD disease.
