摘  要:目的:分析探讨羟乙基淀粉130/0.4氯化钠注射液致过敏性休克的临床特点和救治中存在的问题,为临床安全用药提供参考.方法:检索建库至2022年知网、PubMed等主要数据库中收录的羟乙基淀粉130/0.4氯化钠注射液致过敏性休克病例报告,对过敏性休克性别、年龄、临床表现、过敏史等内容进行统计分析.结果:共得到文献16篇,共17例患者,其中死亡1例.12例患者无过敏史,休克多发生在给药的20 min内,心血管系统反应是主要的临床表现,且救治过程中肾上腺素使用率普遍不高.结论:羟乙基淀粉130/0.4氯化钠注射液致过敏性休克累及多个器官或系统;过敏史对其预示作用有限;医务人员应充分了解该药物的机理特性以及临床特点,提高鉴别准确性;强化对过敏性休克救治药物和流程的掌握,提高救治率.Objective:To analyze and explore the clinical characteristics and problems in the rescue treatment of anaphylaxis caused by hydroxyethyl starch 130/0.4 sodium chloride injection,and to provide reference for the safe clinical use of drugs.Methods:The case reports of anaphylactic shock caused by hydroxyethyl starch 130/0.4 sodium chloride injection included in the main databases,such as CNKI and PubMed,were searched from the establishment to 2022,and the statistical analysis of patients'gender,age,clinical manifestations and allergy history was conducted.Results:A total of 16 literature articles were obtained with 17 patients,including 1 death.12 patients had no history of allergy,shock occurred mostly within 20 minutes of drug administration,and cardiovascular system reaction was the main clinical manifestation.However,the rate of epinephrine use during rescue was generally low.Conclusion:Hydroxyethyl starch 130/0.4 sodium chloride injection causes anaphylaxis involving multiple organs or systems;allergy history has limited playing a foreshadowing role on anaphylaxis effect;medical personnel should fully understand the the pharmacological properties as well as clinical characteristics of this drug to improve the accuracy of identification of allergic reactions and strengthen the mastery of anaphylaxis rescue drugs and procedures to improve the rescue rate.
