摘 要:目的:探讨经皮神经电刺激治疗特发性非梗阻性尿潴留的安全性与有效性.方法:选择2018年9月—2021年10月佳木斯市中心医院诊治的特发性非梗阻性尿潴留患者88例,根据随机平均分配原则分为电刺激组与对照组,各44例.对照组给予常规治疗,电刺激组在对照组基础上给予经皮神经电刺激治疗,比较两组临床疗效、不良反应发生率、盆腔神经感觉诱发电位波幅及潜伏期变化、血清脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)含量.结果:电刺激组总有效率为100.0%,高于对照组的86.4%(P<0.05);电刺激组不良反应发生率为4.5%,低于对照组的25.0%(P<0.05);电刺激组盆腔神经感觉诱发电位波幅高于对照组(P<0.05),而两组治疗前后潜伏期比较,差异无统计学意义(P>0.05);电刺激组血清BDNF含量高于对照组(P<0.05).结论:经皮神经电刺激治疗特发性非梗阻性尿潴留的效果确切,可改善盆腔神经感觉诱发电位波幅,增加血清BDNF含量,降低不良反应发生概率,具有应用有效性及安全性.Objective:To investigate the safety and efficacy of transcutaneous electrical nerve stimulation in the treatment of idiopathic non-obstructive urinary retention.Methods:A total of 88 patients with idiopathic non-obstructive urinary retention who were diagnosed and treated in Jiamusi Central Hospital from September 2018 to October 2021 were selected and divided into an electrical stimulation group and a control group according to the principle of random average distribution,with 44 cases in each group.The control group was given routine treatment,and the electrical stimulation group was given transcutaneous electrical nerve stimulation on the basis of the control group.The clinical efficacy,incidence of adverse reactions,changes in the amplitude and latency of pelvic nerve sensory evoked potentials,and serum brain-derived neurotrophic factor(BDNF)levels were compared between the two groups.Results:The total effective rate in the electrical stimulation group was 100.0%,which was higher than 86.4%in the control group(P<0.05).The incidence of adverse reactions in the electrical stimulation group was 4.5%,which was lower than 25.0%in the control group(P<0.05).The amplitude of pelvic nerve sensory evoked potentials was higher than that in the control group(P<0.05),while the latency between the two groups before and after treatment showed no significant difference(P>0.05).The serum BDNF content in the electrical stimulation group was higher than that in the control group(P<0.05).Conclusion:Transcutaneous electrical nerve stimulation is effective in the treatment of idiopathic non-obstructive urinary retention.It can improve the amplitude of pelvic nerve sensory evoked potentials,increase the content of serum BDNF,and reduce the incidence of adverse reactions.It is effective and safe.
