双切口双钢板与解剖钢板内固定治疗复杂胫骨平台骨折的临床效果

摘　　要：目的:探讨双切口双钢板内固定(DIDPIF)与解剖钢板内固定治疗复杂胫骨平台骨折(CTPF)的效果.方法:选取2019年8月—2021年3月我院收治的106例CTPF患者,按随机数字表法分为对照组和观察组,各53例.对照组采取解剖钢板内固定治疗,观察组采取DIDPIF治疗,比较两组手术情况、术后恢复情况、膝关节功能[美国特种外科医院(HSS)评分]及并发症发生率.结果:两组手术时间、切口长度、术中出血量比较,差异无统计学意义(P>0.05);观察组骨折愈合、完全负重、膝关节锻炼时间均短于对照组(P<0.05);观察组术后12个月HSS评分高于对照组(P<0.05);观察组并发症发生率为3.77%,低于对照组的15.09%(P<0.05).结论:DIDPIF与解剖钢板内固定均可治疗CTPF,但DIDPIF能缩短CTPF患者术后康复所需时间,改善患者膝关节功能,降低并发症发生率,安全性更高.Objective:To investigate the clinical effect of double incision double plate internal fixation(DIDPIF)and anatomical plate internal fixation in the treatment of complex tibial plateau fractures(CTPF).Methods:A total of 106 patients with CTPF treated in our hospital from August 2019 to March 2021 were randomly divided into control group and observation group,with 53 cases in each group.The control group was treated with anatomical plate internal fixation,and the observation group was treated with DIDPIF.The operation,postoperative recovery,knee function[American hospital for special surgery(HSS)score]and the incidence of complications were compared between the two groups.Results:There was no significant difference in operation time,incision length and intraoperative bleeding between the two groups(P>0.05).Fracture healing,complete weight bearing and knee joint exercise time in the observation group were shorter than those in the control group(P<0.05).HSS score at 12 months after operation in observation group was higher than that in control group(P<0.05).The incidence of complications in the observation group was 3.77%,which was lower than that 15.09%in the control group(P<0.05).Conclusion:Both DIDPIF and anatomical plate internal fixation can treat CTPF,but DIDPIF can shorten the time required for postoperative rehabilitation of CTPF patients,improve knee joint function,reduce the incidence of complications,and has higher safety.
