饮水法+二甲硅油对无痛胃镜检查图像质量的有效性和经济性评价

摘　　要：目的探讨饮水法+二甲硅油在无痛胃镜检查前准备中的应用价值。方法选取行无痛胃镜检查患者114例,按照随机数字表法分为二甲硅油单用组、饮水联合组和链霉蛋白酶联合组,每组38例。二甲硅油单用组检查前口服二甲硅油,饮水联合组口服0.9%氯化钠溶液+二甲硅油,链霉蛋白酶联合组口服链霉蛋白酶+二甲硅油。比较3组患者胃黏膜清晰度评分、微小病灶检出率、内镜检查舒适度问卷评分以及经济学指标的差异。结果二甲硅油单用组胃体、胃底、胃窦、十二指肠等部位黏膜清晰度评分高于饮水联合组和链霉蛋白酶联合组,红斑、糜烂、出血点等微小病灶检出率低于饮水联合组和链霉蛋白酶联合组,差异均有统计学意义(P<0.05);饮水联合组和链霉蛋白酶联合组上述指标差异无统计学意义(P>0.05)。二甲硅油单用组冲洗次数、检查时间多于饮水联合组和链霉蛋白酶联合组,二甲硅油单用组和饮水联合组检查费低于链霉蛋白酶联合组,差异均有统计学意义(P<0.05);饮水联合组和链霉蛋白酶联合组冲洗次数、检查时间以及二甲硅油单用组和饮水联合组检查费差异均无统计学意义(P>0.05)。结论无痛胃镜检查前使用饮水+二甲硅油可有效提高胃黏膜清晰度,并能减轻患者经济负担和医护人员工作量。Objective To investigate the application of water intake combined with dimethicone in patients undergoing painless gastroscopy.Methods A total of 114 patients undergoing painless gastroscopy were randomized into dimethicone group,water intake combination group and chain protease combination group,with 38 cases in each group.The patients in dimethicone group,water intake combination group and chain protease combination group were respectively treated by dimethicone,0.9%sodium chloride solution combined with dimethicone and chain protease combined with dimethicone before gastroscopy.Gastric mucosal visibility scores,detection rate of small lesions,endoscopic operation comfort questionnaire scores and economic index were compared among three groups.Results The mucosal visibility scores of gastric body,fundus,antrum and duodenum were significantly higher in the dimethicone group compared with those in the water intake combination group and chain protease combination group,while the detection rate of small lesions like erythema,erosion,and bleeding point were significantly lower in the former group(P<0.05).However,there were no significant differences in the abovementioned indexes between water intake combination group and chain protease combination group(P>0.05).The examination time and number of flushing were significantly higher in the dimethicone group compared with the water intake combination group and chain protease combination group,while the examination fees were significantly lower in the dimethicone group and water intake combination group compared with the chain protease combination group(P<0.05).Moreover,there were no significant differences in the examination time and number of flushing between water intake combination group and chain protease combination group,and there was no significant difference in the examination fees between dimethicone group and water intake combination group(P>0.05).Conclusion Application of water intake combine with dimethicone for painless gastroscopy can effectively improve g
