PICC置管后局部皮肤过敏患者使用氯己定醇的疗效观察

摘　　要：目的探讨氯己定醇对经外周静脉穿刺中心静脉置管(PICC)置管后局部皮肤过敏患者的疗效。方法选取2020年1月至2021年1月收治的PICC置管后局部皮肤过敏患者120例,随机数表法分为A组(60例,采用2%葡萄糖酸氯己定醇消毒)和B组(60例,采用0.5%聚维酮碘消毒),疗程均为7 d;比较2组治疗前后过敏性皮炎分度、12项瘙痒症严重程度量表(12-PSS)评分,临床疗效及PICC并发症。结果治疗前,2组患者过敏性皮炎分度、12-PSS评分差异无统计学意义(P>0.05);治疗后,A组的过敏性皮炎分度、12-PSS评分低于B组,差异均有统计学意义(P<0.05);A组总有效率高于B组,差异有统计学意义(P<0.05);2组患者的总并发症发生率差异无统计学意义(P>0.05)。结论PICC置管后局部皮肤过敏患者采用2%葡萄糖酸氯己定醇进行皮肤消毒的疗效较佳,可有效改善患者过敏性皮炎红斑及瘙痒症状。Objective To explore the efficacy of chlorhexidine alcohol in patients with local skin allergy after catheterization of the peripherally inserted central catheter(PICC).Methods A total of 120 patients with local skin allergy after PICC catheterization from January 2020 to January 2021 were randomly assigned into group A(60 cases were disinfected with 2%chlorhexidine gluconate alcohol)and group B(60 cases were disinfected with 0.5%povidone iodine)for 7 days.The degree of allergic dermatitis,12-item pruritus severity scale(12-PSS),clinical efficacy and PICC complications were compared between the two groups before and after treatment.Results The degree of allergic dermatitis and 12-PSS scores before treatment were comparable between groups(P>0.05),which were significantly lower in group A than those of group B after treatment(P<0.05).The total effective rate in group A was significantly higher than that in group B(96.67%vs 80.00%;P<0.05,respectively).There was no significant difference in the total complication rate of PICC between groups(3.33%vs 10.00%;P>0.05,respectively).Conclusion Compared with 0.5%povidone iodine,the efficacy of 2%chlorhexidine alcohol for skin disinfection in patients with local skin allergy after PICC catheterization is better,which can effectively improve the symptoms of allergic dermatitis,erythema and pruritus.
