急性心肌梗死患者NLRP3、IL-18及IL-1β水平与血栓积分、肌钙蛋白的关系分析

摘　　要：目的探讨急性心肌梗死患者核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)、白细胞介素18(IL-18)及白细胞介素1β(IL-1β)水平与血栓积分、肌钙蛋白的关系。方法选择2020年1月至2021年5月于河北中石油中心医院收治的急性心肌梗死患者106例作为病例组,另选取同期我院健康体检者95例作为对照组。分析两组在血清NLRP3、IL-18及IL-1β水平变化情况,并分析其与血栓积分、肌钙蛋白之间的关系。结果病例组血清NLRP3、IL-18及IL-1β水平显著高于对照组,差异有统计学意义(P<0.05);按照病变狭窄累积左冠状动脉(冠脉)的支数分为单支组26例,双支组45例,三支组35例。其中单支组血清NLRP3、IL-18及IL-1β水平显著低于双支组和三支组,双支组血清NLRP3、IL-18及IL-1β水平显著低于三支组,差异有显著统计学意义(P<0.01);病例组血栓积分、肌钙蛋白水平显著高于对照组,差异有统计学意义(P<0.05);单支组血栓积分、肌钙蛋白水平显著低于双支组和三支组(P<0.01);双支组血栓积分、肌钙蛋白水平显著低于三支组(P<0.01);相关性分析结果显示,急性心肌梗死患者血清NLRP3、IL-18及IL-1β均和血栓积分、肌钙蛋白之间呈正相关(P<0.05)。结论急性心肌梗死患者NLRP3、IL-18及IL-1β水平表达异常,且与血栓积分、肌钙蛋白之间存在相关性,可能协同参与急性心肌梗死的发生及发展。Objective To investigate the relationship among NOD-like receptor thermal protein domain associated protein 3(NLRP3),interleukin-18(IL-18),interleukin-1β(IL-1β),thrombotic integrals and troponin in patients with acute myocardial infarction(AMI).Methods AMI patients(n=106)were chosen into case group,and meanwhile controls with physical examinations(n=95)were chosen into control group from Central Hospital of China National Petroleum Corporation in Hebei Province from Jan.2020 to May 2021.The changes of NLRP3,IL-18 and IL-1βwere observed,and the relationship among them and thrombotic integrals and troponin was analyzed.Results The levels of NLRP3,IL-18 and IL-1βwere significantly higher in case group than those in control group(P<0.05).The levels of NLRP3,IL-18 and IL-1βwere significantly lower in 1-vessel lesion group than those in 2-vessel lesion group and 3-vessel lesion group,and were significantly lower in 2-vessel lesion group than that in 3-vessel lesion group(P<0.01).The thrombotic integrals and troponin level were significantly higher in case group than those in control group(P<0.05).The thrombotic integrals and troponin level were significantly lower in 1-vessel lesion group than those in 2-vessel lesion group and 3-vessel lesion group,and were significantly lower in 2-vessel lesion group than that in 3-vessel lesion group(P<0.01).The results of correlation analysis showed that NLRP3,IL-18 and IL-1βwere positively correlated to thrombotic integrals and troponin(P<0.05).Conclusion The expressions of NLRP3,IL-18 and IL-1βare abnormal,and correlated to thrombotic integrals and troponin in AMI patients,which may be synergistically involved in the occurrence and development of AMI.
