尿mALB、尿沉渣小圆上皮细胞检测在过敏性紫癜肾损害诊断中的应用价值

摘　　要：目的:探讨尿沉渣小圆上皮细胞、尿微量蛋白(mALB)检测在过敏性紫癜肾损害诊断中的应用价值。方法:将2020年3月至2021年8月山东颐养健康集团新泰协庄医院收治的74例过敏性紫癜患者作为观察组,将73例健康体检者作为对照组,检测两组的尿mALB、微球蛋白、N-乙酰-β-D-葡萄糖苷酶(NAG)、血清胱抑素C及尿沉渣小圆上皮细胞水平,并对比结果。结果:观察组的尿mALB、微球蛋白、NAG、血清胱抑素C水平均高于对照组,尿沉渣小圆上皮细胞个数多于对照组(P<0.05)。观察组的尿mALB、微球蛋白、NAG、血清胱抑素C及尿沉渣小圆上皮细胞阳性率均高于对照组(P<0.05)。观察组各指标检测的特异度及灵敏度均高于对照组(P<0.05)。结论:尿mALB、微球蛋白、NAG、血清胱抑素C及尿沉渣小圆上皮细胞水平在过敏性紫癜肾损害中均具有高表达性。检测上述指标可提高过敏性紫癜肾损害的诊断效能。Objective:To investigate the diagnostic value of urinary sludge small ro und epithelial cells and urinary microprotein(mALB)in the diagnosis of renal damage in henoch purpura.How:The levels of urinary mALB,microglobulin,n-acetyl-β-D-glucosidase(NAG),serum cystatin C and urinary small round epithelial cells of the two groups were detected in 74 patients with anaphylactoid purpura admitted to Xintai Xizhuang Hospital of Shandong Nursing and Health Group from March 2020 to August 2021 as the observation group,and 73 healthy subjects as the control group.And compare the results.Results:The levels of urinary mALB,microglobulin,NAG and cystatin C in observation group were higher than those in control group,and the number of small round epithelial cells of urinary sludge was higher than that in control group(P<0.05).The positive rates of urinary mALB,microglobulin,NAG,cystatin C and small round epithelial cells of urinary sludge in observation group were higher than those in control group(P<0.05).The specificity and sensitivity of each index in the observation group were higher than those in the control group(P<0.05).Conclusion:Urinary mALB,microglobulin,NAG,serum cystatin C and small round epithelial cells of urinary sludge have high expression in renal injury of henoch purpura.Detection of these indexes can improv e the diagnostic efficiency of renal damage in henoch purpura.
