摘  要:目的探究超声造影及剪切波弹性成像与乳腺癌生物学预后因子核增殖相关抗原(nuclear-associatedantigen67,Ki-67)、人类表皮生长因子受体2(human epidermal growth factor receptor 2,HER2)、雌激素受体(estrogen receptor,ER)相关性。方法回顾性分析2019年6月至2021年6月义乌復元第一医院收治的60例乳腺癌患者作为观察组,选取同期接受超声检查的健康者56例作为对照组,两组均进行超声检查,采用受试者操作特征(receiver operator characteristic,ROC)曲线分析峰值强度(peak intensity,Peak)、造影剂平均通过时间(mean transit time,MTT)、肿块弹性最小值(minimum elasticity value,Emin)、弹性平均值(average elasticity value,Emean)、弹性最大值(maximum elasticity value,Emax)预测诊断效能。同时对观察组进行ER、HER2、Ki-67检测,采用Spearman相关性分析对生物学预后因子与超声定量参数相关性进行检验。结果观察组的Emean显著低于对照组(P<0.05),Emax、Emin、Peak、MTT显著高于对照组(P<0.05)。ROC曲线分析显示,Emean、Emax、Emin、Peak、MTT诊断乳腺癌的曲线下面积分别为0.830、0.822、0.900、0.892、0.885。Ki-67与Emean、Emax、Peak、MTT呈正相关(P<0.05);ER与Emean、Emax、Emin、MTT呈负相关(P<0.05);HER-2与Emean、Emax、Emin、Peak、MTT呈正相关(P<0.05)。结论超声造影、剪切波弹性成像定量值与乳腺癌生物学预后因子具有一定相关性,可作为疾病诊断、预后评估重要依据。Objective To explore the relationship between contrast-enhanced ultrasound and shear wave elastography and breast cancer biological prognostic factors nuclear proliferation-related antigen(Ki-67),human epidermal growth factor receptor 2(HER2),estrogen receptor(ER).Methods A retrospective analysis of 60 breast cancer patients admitted to Yiwu Fuyuan First Hospital from June 2019 to June 2021 was performed as the observation group,and 56 healthy individuals who underwent ultrasound examination during the same period were selected as the control group,all of whom underwent ultrasound examinations.The peak intensity(Peak),the mean transit time(MTT)of the contrast agent,the minimum elasticity value(Emin),the average elasticity value(Emean)and the maximum elasticity value(Emax)of the tumor were analyzed by receiver operator characteristic(ROC)curve.At the same time,ER,HER2 and Ki-67 tests were performed in the observation group,and the correlation between biological prognostic factors and quantitative ultrasound parameters was tested by Spearman correlation analysis.Results Emean in the observation group was significantly lower than that in the control group(P<0.05),and Emax,Emin,Peak and MTT were significantly higher than those in the control group(P<0.05).ROC curve analysis showed that the area under the curve for the diagnosis of breast cancer was 0.830,0.822,0.900,0.892 and 0.885 for Emean,Emax,Emin,Peak and MTT,respectively.Ki-67 was positively correlated with Emean,Emax,Peak and MTT(P<0.05);ER was negatively correlated with Emean,Emax,Emin and MTT(P<0.05);HER-2 was positively correlated with Emean,Emax,Emin,Peak and MTT(P<0.05).Conclusion The quantitative values of contrast-enhanced ultrasound and shear wave elastography have a certain correlation with biological prognostic factors of breast cancer,which can be used as an important basis for disease diagnosis and prognosis evaluation.
