三种结核潜伏感染试验在HIV感染人群中的应用评价

摘　　要：目的评价AIMTB和T-SPOT检测方法在人类免疫缺陷病毒(human immunodeficiency virus,HIV)感染人群中结核潜伏感染的诊断效能。方法收集2022年9~10月在广西壮族自治区疾病预防控制中心门诊就诊的HIV感染者的血样标本,使用雷德AIMTB、迪奥T-SPOT和凯杰QFT三种方法进行检测,以QFT作为参照对检测结果进行诊断效能评价。结果AIMTB方法检出阳性率为54.6%,敏感度、特异性分别为84.72%、76.19%;T-SPOT方法检出阳性率为57.6%,敏感度和特异性分别为88.89%、73.02%。AIMTB方法和T-SPOT方法与QFT方法比较结果具有一致性(P<0.001)。结论AIMTB方法和T-SPOT方法均具有较高的阳性检出率,与QFT方法的一致性较好,可加大样本量进一步验证及推广。Objective To evaluate the effectiveness of AIMTB and T-SPOT tests in the diagnosis of latent tuberculosis infection in human immunodeficiency virus(HIV)infected population.Methods Collect blood samples from HIV-infected patients in the outpatient department of Guangxi Zhuang Autonomous Region Center for Disease Control and Prevention from September to October 2022.The blood samples were tested by three methods:AIMTB,T-SPOT and QFT test.The test results were evaluated by diagnostic tests with QFT as a reference.Results The positive rate of AIMTB was 54.6%,the sensitivity and specificity were 84.72%and 76.19%,respectively.The positive rate of T-SPOT was 57.6%,the sensitivity and specificity were 88.89%and 73.02%,respectively.AIMTB and T-SPOT were consistent with QFT test(P<0.001).Conclusion AIMTB and T-SPOT have high positive detection rates,and they are consistent with QFT test,which can be further verified and popularized by increasing the samples.
