益气养阴解毒汤加味治疗早期糖尿病肾病的疗效观察

摘　　要：目的探讨益气养阴解毒汤加味辅助治疗早期糖尿病肾病(diabetic nephropathy,DN)气阴两虚夹瘀证的疗效。方法选取2019年1月至2022年1月于乐清市第二人民医院就诊的早期DN患者80例,根据随机数字表法将其分为治疗组和对照组,每组各40例。两组患者均依据指南给予常规干预措施,对照组患者口服坎地沙坦酯片,治疗组患者在对照组基础上服用益气养阴解毒汤加味;两组患者均治疗12周。比较两组患者的血糖相关指标、肾功能、气阴两虚夹瘀证单项指标评分、临床疗效及血清肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)与白细胞介素-6(interleukin-6,IL-6)水平。结果治疗后,两组患者的空腹血糖(fasting blood glucose,FPG)、餐后2h血糖(2h postprandial blood glucose,2hPG)、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、24h尿蛋白定量、尿白蛋白/肌酐比、血尿素氮、血肌酐、气阴两虚夹瘀证各项指标评分及血清TNF-α、IL-6水平均显著低于本组治疗前(P<0.05),治疗组患者的FPG、2hPG、Hb A1c、24h尿蛋白定量、尿白蛋白/肌酐比、血尿素氮、血肌酐、气阴两虚夹瘀证各项指标评分及血清TNF-α、IL-6水平均显著低于对照组(P<0.05);治疗组患者的总有效率显著高于对照组(χ^(2)=4.242,P<0.001)。结论在常规干预措施的基础上,益气养阴解毒汤加味辅助治疗早期DN气阴两虚夹瘀证的疗效更优,可进一步控制患者的血糖水平,改善肾功能及中医证候,减轻微炎症状态。Objective To investigate the effect of auxiliary treatment of Yiqi Yangyin Jiedu decoction on early diabetic nephropathy(DN)with syndrome of deficiency of both qi and yin combined with stasis.Methods Eighty patients with early DN admitted to the Second People’s Hospital of Yueqing from January 2019 to January 2022 were selected and divided into treatment group and control group according to random number table method,with 40 cases in each group.Patients in two groups were given routine intervention measures according to the guidelines.Patients in control group were given candesartan cilexetil,and patients in treatment group were given Yiqi Yangyin Jiedu decoction on the basis of control group.Two groups were treated for 12 weeks.Blood glucose related indexes,renal function,single index scores of syndrome of deficiency of both qi and yin combined with stasis,the clinical efficacy,and serum levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were compared between the two groups.Results After treatment,fasting blood glucose(FPG),2h postprandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c),24h urinary protein,urinary albumin/creatinine ratio,blood urea nitrogen,blood creatinine,single index scores of syndrome of deficiency of both qi and yin combined with stasis,serum TNF-αand IL-6 levels in two groups were significantly lower than before treatment(P<0.05).The levels of FPG,2hPG,HbA1c,24h urinary protein,urinary albumin/creatinine ratio,blood urea nitrogen,blood creatinine,single index scores of syndrome of deficiency of both qi and yin combined with stasis and serum TNF-αand IL-6 in treatment group were significantly lower than those in control group(P<0.05).The total effective rate in treatment group was significantly higher than that in control group(χ^(2)=4.242,P<0.05).Conclusion On the basis of conventional intervention measures,auxiliary treatment of Yiqi Yangyin Jiedu decoction on early DN with syndrome of deficiency of both qi and yin combined with stasis has a better efficacy,can furt
