痔洗三号剂熏洗坐浴联合手术治疗混合痔伴肛周湿疹(湿热下注证)的效果

摘　　要：目的探讨痔洗三号剂熏洗坐浴联合手术治疗对混合痔伴肛周湿疹(湿热下注证)的临床疗效。方法收集2021年6月至2022年6月常德市第一中医医院住院的混合痔伴肛周湿疹患者,按随机数表法分为对照组和治疗组,每组各36例,对照组给予高锰酸钾熏洗坐浴联合手术,治疗组给予痔洗三号剂熏洗坐浴联合手术,治疗2周后随访6个月记录恢复情况。统计肛周湿疹局部症状评分(含疗效评价)、嗜酸性粒细胞值、术后恢复、药物不良反应、复发及生活质量评分。结果治疗组疗效评价有效率为97.2%,对照组80.6%,差异有统计学意义(P<0.05)。两组肛周湿疹局部症状评分治疗后均降低,治疗组降低更明显,差异有统计学意义(P<0.05)。治疗组术后恢复包括切口愈合时间及皮损消退时间,研究组均低于对照组(P<0.05)。两组不良反应发生率均为2.78%,差异无统计学意义(P>0.05)。肛周湿疹复发率治疗组为5%,对照组为22%,差异有统计学意义(P<0.05)。治疗组生活质量评分均高于对照组,差异有统计学意义(P<0.05)。结论痔洗三号剂熏洗坐浴联合手术治疗混合痔伴肛周湿疹(湿热下注证)疗效显著,可加快术后恢复,降低复发率,且安全有效,同时可提升患者生活质量。Objective To investigate the clinical efficacy of mixed hemorrhoids with perianal eczema(syndrome of damp invasion of lower energizer)treated by No.3 hemorrhoids lotion fumigation and sitz bath combined with surgery.Methods The patients of mixed hemorrhoids with perianal eczema hospitalized in No.1 Traditional Chinese Medicine Hospital in Changde from June 2021 to June 2022 were collected and divided into the control group(n=36)and the treatment group(n=36)according to the random number table method.The control group was treated with potassium permanganate fumigation and sitz bath combined with surgery,while the treatment group was treated with No.3 hemorrhoids lotion fumigation and sitz bath combined with surgery.After 2 weeks of treatment,the recovery status was recorded at a follow up of 6 months.The scores of local symptoms of perianal eczema(including therapeutic efficacy evaluation),eosinophil values,postoperative recovery,adverse drug reactions(ADRs),relapse and quality of life(QoL)were statistically analyzed.Results The effective rate of therapeutic efficacy evaluation in the treatment group was 97.2%,while that in the control group was 80.6%,with statistically significant difference(P<0.05).The scores of local symptoms of perianal eczema in both groups decreased after treatment,and the decrease was more remarkable in the treatment group,with statistically significant difference(P<0.05).The postoperative recovery of the treatment group included the time of incision healing and the time of skin lesion regression,and those in the study group were lower than those in the control group(P<0.05).The incidences of ADRs in both groups were 2.78%,without statistically significant difference(P>0.05).The relapse rate of perianal eczema was 5%in the treatment group and 22%in the control group(P<0.05).The QoL scores in the treatment group were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion The therapeutic efficacy of mixed hemorrhoids with perianal eczema(syndrome
