摘 要:目的:分析女性盆底功能障碍性疾病诊断过程中经会阴三维盆底超声和磁共振成像(MRI)检查的应用效果。方法:选取平原县第一人民医院2021年1月—2022年6月收治的50例盆底功能障碍性疾病患者为研究组,将同期在我院接受健康体检的40名女性纳入对照组,均接受MRI与经会阴三维盆底超声检查,分析其诊断结果。结果:结合临床综合诊断结果进行判断,盆底超声与MRI诊断盆底功能障碍性疾病的准确率分别为92.00%和96.00%,差异不显著(P>0.05)。经MRI检查,研究组膀胱颈、肛管直肠连接以及子宫颈下降距离均显著大于对照组(P<0.01)。超声影像比较,研究组静息、张力以及缩肛状态下的盆膈裂孔前后径均显著大于对照组,肛提肌厚度显著小于对照组,子宫后位疾病发生率占比显著高于对照组(P<0.01)。结论:经会阴三维盆底超声与MRI检查对该疾病均具有较高的检出率,均能够较好地反映患者盆底功能与结构,但是三维超声操作更为简便,且费用更低,所以临床可将该方法作为首选。Objective To analyze the diagnostic effect of transperineal three-dimensional ultrasound pelvic floor ultrasound and magnetic resonance imaging(MRI)in female pelvic floor dysfunction diseases.Methods 50 patients with pelvic floor dysfunction admitted to the First People's Hospital of Pingyuan County from January 2021 to June 2022 were selected as the study group,and 40 female patients who received physical examination in our hospital during the same period were included in the control group.All of them received MRI and transperineal three-dimensional pelvic floor ultrasound examination,and their diagnosis results were analyzed.Results According to the clinical comprehensive diagnosis results,the diagnostic accuracy rates of pelvic floor ultrasound and MRI were 92.00%and 96.00%,respectively,with no significant difference(P>0.05).After MRI examination,the distance of bladder neck,anorectal connection and cervical descent in the study group was significantly greater than that in the control group(P<0.01).Compared with the control group,the anterior and posterior diameters of the pelvic diaphragmatic hiatus in the study group were significantly larger at rest,tension and anal constriction,the thickness of the levator ani muscle was significantly smaller,and the incidence of posterior uterine disease was significantly higher in the study group than in the control group(P<0.01).Conclusion Transperineal three-dimensional pelvic floor ultrasound and MRI have a high detection rate for this disease,and both can better reflect the function and structure of the patient's pelvic floor.However,three-dimensional ultrasound is more simple to operate,and the cost is lower,so this method can be used as the first choice in clinic.
