摘 要:目的:探讨床旁超声心动图评估重度颅脑损伤(severe craniocerebral injury,SCI)患者左心功能改变的应用价值。方法:选取2021年7月—2022年12月江阴市人民医院收治的27名SCI患者(SCI组)和30名正常体检人员(对照组),均行床旁超声心动图检查,比较两组左心结构参数、左室收缩功能参数和左室舒张功能参数之间的差异。结果:在左室结构参数中,SCI组LAD、LVDs均大于对照组,差异有统计学意义(P<0.05),两组LVDd、IVS、LVPW参数差异无统计学意义(P>0.05);在左室收缩功能参数中,两组LVEDV、CO差异不显著(P>0.05),SCI组LVEF、LVFS、SV均低于对照组,LVESV大于对照组,差异具有统计学意义(P<0.05);在左室舒张功能参数中,SCI组e’低于对照组,E/e’、TR高于对照组,差异具有统计学意义(P<0.05),两组E、A、E/A指标值差异均无统计学意义(P>0.05)。结论:SCI患者左室功能较健康人员会发生改变,应用超声心动图对SCI患者的左心室功能进行评估可为临床诊疗方案提供依据。Objective To investigate the clinical value of bedside echocardiography in evaluating left heart function in patients with severe craniocerebral injury(SCI).Methods From July 2021 to December 2022,27 SCI patients(SCI group)and 30 healthy individuals(control group)were selected for bedside echocardiography examination to compare the differences between the two groups in left ventricular structural parameters,left ventricular systolic function parameters,and left ventricular diastolic function parameters.Results In terms of left ventricular structural parameters,the LAD and LVDs in the SCI group were greater than those in the control group,with statistically significant differences(P<0.05),there were no statistically significant difference in LVDd,IVS,and LVPW parameters between the two groups(P>0.05);In terms of left ventricular systolic function parameters,there were no significant difference in LVEDV and CO between the two groups(P>0.05),LVEF,LVFS,SV in the SCI group were lower than those in the control group,while LVESV was higher than those in the control group,with statistical significance(P<0.05);In terms of left ventricular diastolic function parameters,the SCI group showed a significant decrease in e'compared to the control group,while E/e'and TR increased compared to the control group,with statistically significant differences(P<0.05),there were no statistically significant difference in the values of E,A,and E/A indicators between the groups(P>0.05).Conclusion Patients with severe craniocerebral injury may get changes in left ventricular function compared to healthy individuals.Evaluating the left ventricular function of SCI patients using echocardiography can provide a basis for clinical diagnosis and treatment.
