交联透明质酸钠凝胶联合优思明对超声人工流产术后宫腔恢复的影响

摘　　要：目的:观察交联透明质酸钠凝胶联合优思明预防人工流产术后宫腔粘连效果。方法:回顾性分析2021年1-12月本院行人工流产术女性临床资料,按照术后预防宫腔粘连治疗方式不同分为单一组(优思明)和联合组(交联透明质酸钠凝胶联合优思明),经倾向性匹配评分法排除年龄、孕周等混杂因素,最后两组各获得97例,比较两组术后阴道出血时间、术后阴道流血量,首次月经复潮时间、月经量减少例数,宫腔粘连、白带常规检查异常情况及药物治疗期间不良反应情况。结果:联合组术后阴道出血时间(4.3±1.6d)、月经复潮时间(25.7±5.7d)均短于单一组(5.5±1.7d、28.0±5.5d),术后阴道流血量(5.0±1.2ml)和月经量减少比例(5.1%)均少于单一组(7.0±1.2ml、13.4%),术后宫腔粘连发生率(1.0%)低于单一组(7.2%)(均P<0.05),两组术后白带异常(2.1%、6.2%)及治疗期间药物不良反应发生(2.1%、3.1%)无差异(P>0.05)。结论:交联透明质酸钠凝胶联合优思明可有效预防人工流产术后宫腔粘连发生,利于促进月经恢复,不良反应少、安全可靠。Objective:To observe the effect of cross-linked sodium hyaluronate gel drospirenone-ethinylestradiol pill for preventing intrauterine adhesion of women after induced abortion.Methods:The clinical data of the women who had undergone induced abortion from January 2021to December 2021were analyzed retrospectively.According to the different methods for preventing intrauterine adhesion after abortion,these women were divided into study group(women with treatment of cross-linked sodium hyaluronate gel combined with drospirenone-ethinylestradiol pill)and control group(women with treatment of drospirenone-ethinylestradiol pill alone).The confounding factors,such as age and gestational weeks of the women were excluded by propensity matching score,97patients were included in each group finally.The time and volume of postoperative vaginal bleeding,the time of the first menstrual recovery,the number of women with decreased menstrual volume,the situations of uterine adhesion and leucorrhea routine examination abnormalities,and the adverse reactions during treatment of the patients were compared between the two groups.Results:The postoperative vaginal bleeding time(4.3±1.6d)and the menstrual recovery time(25.7±5.7d)of the patients in the study group were significantly shorter than those(5.5±1.7dand 28.0±5.5d)of the patients in the control group.The postoperative vaginal bleeding amount(5.0±1.2ml)and the percentage of menstrual volume reduction(5.1%)of the patients in the study group were significantly less than those(7.0±1.2ml and 13.4%)of the patients in the control group.The incidence of postoperative intrauterine adhesions(1.0%)of the patients in the study group was significantly lower than that(7.2%)of the patients in the control group(all P<0.05).The rates of postoperative leucorrhea abnormality(2.1%vs.6.2%)and the drug adverse reactions during treatment(2.1%vs.3.1%)of the patients had no significant differences between the two groups(P>0.05).Conclusion:Cross-linked sodium hyaluronate gel combined with drospirenone-
