妊娠期糖尿病线上线下闭环管理模式干预效果

摘　　要：目的:探讨线上线下闭环管理模式对妊娠期糖尿病(GDM)孕妇糖脂代谢、妊娠结局的影响。方法:选取2019年1月-2021年1月在本院规范产前检查并在孕24周后行75g葡萄糖耐量试验确诊为GDM孕妇160例,按干预方式不同分为常规组和干预组各80例,确诊后均常规门诊指导控制血糖,干预组同时进行线上线下闭环管理模式干预,通过线上门诊指导、线下微信群建立,督导患者饮食和运动,动态监测血糖变化并及时指导。孕28~32周时检测两组血糖、糖化血红蛋白、血脂等指标,统计妊娠合并症、不良妊娠结局情况。结果:干预后,两组血糖、糖化血红蛋白、血脂等指标均得到改善,但干预组改善程度优于常规组,妊娠期高血压(6.3)、子痫前期(5.0%)、羊水过多(8.8%)、胎膜早破(10.0%)发生率均低于常规组(17.5%、15.0%、21.3%、23.8%),不良妊娠结局发生低于常规组(均P<0.05)。结论:线上线下闭环管理模式应用于GDM管理,可提高患者的主观能动性,利于医生及时督导,有效地改善了GDM患者的血糖血脂水平,降低了妊娠并发症发生,改善母婴结局。Objective:To explore the online and offline closed loop management model for intervening pregnant women with gestational diabetes mellitus(GDM)on their glycolipid metabolism and pregnancy outcomes.Methods:160pregnant women diagnosed with GDM by the 75g glucose tolerance test after 24gestational weeks during standardized prenatal examination were selected and were divided into two groups(80cases in each group)according to the different intervention methods from January 2019to January 2021.These women all had received routine outpatient guidances for controlling their abnormal blood glucose level.The women in the experimental group were given the online and offline closed loop management model intervention additionally,including the diet and exercise supervised,the blood glucose level changes monitored dynamically,and guidance timely by online outpatient and offline wechat group established.The levels of blood glucose,glycosylated hemoglobin,and blood lipid of the women in the two groups during 28to 32 gestational weeks were detected.The pregnancy complications and the adverse pregnancy outcomes of the women in the two groups were analyzed.Results:The levels of blood glucose,glycosylated hemoglobin,and blood lipid of the women in the two groups after intervention had been improved,but the improvement degree of which of the women in the experimental group were significantly better than those of the women in the control group.The incidences of hypertension during pregnancy(6.3%),preeclampsia(5.0%),hyperamniotic fluid(8.8%),and premature rupture of membranes(10.0%)of the women in the experimental group were significantly lower than those(17.5%,15.0%,21.3%,and 23.8%)of the women in the control group.The incidence of adverse pregnancy outcomes of the women in the experimental group was significantly was lower than that of the women in the control group(all P<0.05).Conclusion:The application of the online and offline closed loop management mode for intervening the pregnant women with GDM can significantly improve their s
