摘　　要：目的探讨慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者树突细胞(dendritic cell,DC)、CD4^(+)叉头状转录因子-3(forkhead transcription facter3,Foxp3)T细胞介导的免疫应答变化及其临床转归。方法以2019年1月至2020年12月沧州市人民医院收治的120例COPD患者作为观察组,COPD稳定期患者78例,加重期患者42例,同时挑选我院健康体检的120例正常志愿者为对照组。采用流式细胞仪检测患者的非组织悬液中的CD4^(+)Foxp3^(+)T细胞以及成熟DC细胞水平,分别对比分析观察组与对照组、稳定期组与加重期组、生存组与死亡组患者的CD4^(+)Foxp3^(+)T细胞以及成熟DC细胞水平。通过线性相关分析CD4^(+)Foxp3^(+)T细胞以及成熟DC细胞水平与患者生存情况以及严重程度的相关性。通过ROC曲线分析CD4^(+)Foxp3^(+)T细胞以及成熟DC细胞水平对于不良预后的预测效能。结果与对照组相比,观察组患者的CD4^(+)Foxp3^(+)T细胞以及成熟DC细胞水平更低,差异均具有统计学意义[%:(2.32±0.36)比(1.45±0.25),(4.09±1.02)比(2.15±0.78),t值分别为21.74和16.55,P值均<0.05]。与加重期患者相比,稳定期患者的CD4^(+)Foxp3^(+)T细胞以及成熟DC水平更高,差异具有统计学意义[%:(0.26±0.08)比(2.09±0.85),(1.41±0.52)比(2.55±0.96),t值分别为18.86和8.44,P值均<0.05]。与死亡组相比,生存组患者的CD4^(+)Foxp3^(+)T细胞以及成熟DC细胞水平更高,差异具有统计学意义[%:(0.37±0.12)比(2.01±0.77),(1.88±0.28)比(2.29±0.56),t值分别为20.07和4.94,P值均<0.05]。通过线性相关性分析,患者的严重程度与生存情况与CD4^(+)Foxp3^(+)T细胞以及成熟DC细胞水平呈现负相关(r=-0.37,-0.46,-0.46,-0.59,P值均<0.05)。ROC曲线分析表明CD4^(+)Foxp3^(+)T细胞以及成熟DC细胞水平对于不良预后具有预测效能(P<0.05)。结论COPD患者DC细胞、CD4^(+)Foxp3^(+)T细胞与疾病的进展以及患者的预后呈现相关性。Objective To study the changes and clinical outcome of immune response mediated by DC cells and CD4^(+)Foxp3^(+)T cells in patients with COPD.Methods Total of 120 patients with COPD admitted to Cangzhou People's Hospital from January 2019 to December 2020 were selected as the observation group,including 78 patients with stable COPD and 42 patients with exacerbation COPD.In addition,120 healthy volunteers in our hospital were selected as the control group.The levels of CD4^(+)Foxp3^(+)T cells and mature DC cells in the patients'non-tissue suspension were detected by flow cytometry,and the levels of CD4^(+)Foxp3^(+)T cells and mature DC cells in the observation group and the control group,the stable group and the exacerbation group,the survival group and the death group were compared and analyzed.Linear correlation was used to analyze the correlation between the level of CD4^(+)Foxp3^(+)T cells and mature DC cells and the survival and severity of patients.The ROC curve was used to analyze the predictive effect of CD4^(+)Foxp3^(+)T cells and mature DC cells on poor prognosis.Results Compared with the control group,the levels of CD4^(+)Foxp3^(+)T cells and mature DC cells in the observation group were significantly lower[%:(2.32±0.36)vs(1.45±0.25),(4.09±1.02)vs(2.15±0.78),t values were 21.74 and 16.55 respectively,both P values<0.05].Compared with patients in exacerbation stage,the level of CD4^(+)Foxp3^(+)T cells and mature DC in stable patients was significantly higher[%:(0.26±0.08)vs(2.09±0.85),(1.41±0.52)vs(2.55±0.96),t values were 18.86 and 8.44 respectively,both P values<0.05].Compared with the death group,the levels of CD4^(+)Foxp3^(+)T cells and mature DC cells in the survival group were significantly higher[%:(0.37±0.12)vs(2.01±0.77),(1.88±0.28)vs(2.29±0.56),t values were 20.07 and 4.94 respectively,both P values<0.05].Through correlation analysis,the severity and survival of patients were negatively correlated with the levels of CD4^(+)Foxp3^(+)T cells and mature DC cells(r values were-0.37,-0.4
