摘  要:目的 从肝细胞自噬与凋亡探讨针灸对腹腔注射顺铂致肝损伤模型小鼠的作用及机制。方法 40只SPF级昆明雄性小鼠随机分为空白组、模型组、针刺组、艾灸组,每组10只。腹腔注射顺铂建立肝损伤模型,针刺组和艾灸组小鼠分别选取“大椎”“肝俞”“肾俞”“足三里”施予针刺、艾灸治疗,每日1次,连续5 d。采用HE染色观察小鼠肝脏组织病理变化,ELISA法检测各组小鼠血清中谷丙转氨酶(alanine aminotransferase,ALT)、谷草转氨酶(aspartate aminotransferase,AST)和谷氨酸脱氢酶(glutamic dehydrogenase,GLDH)的含量,Western blot法检测肝组织中自噬相关的微管相关蛋白1轻链3-Ⅱ型(microtubule associated protein 1light chain 3-Ⅱ,LC3-Ⅱ)、Bcl-2相互作用蛋白(bcl-2 interacting coiled-coil protein 1,Beclin1)及促凋亡蛋白Bc12/腺病毒E1B19kD相互作用蛋白3(bc12/adenovirusE1B 19kDa interacting protein3,BNIP3)的表达。结果 与空白组比较,模型组小鼠血清中ALT、AST、GLDH含量升高,肝组织中LC3-Ⅱ、Beclin1、BNIP3蛋白表达升高(P<0.05);与模型组比较,针刺组和艾灸组小鼠血清中ALT、AST、GLDH含量降低,肝组织中LC3-Ⅱ、Beclin1蛋白表达升高,BNIP3蛋白表达明显降低(P<0.05);与针刺组比较,艾灸组小鼠肝组织中LC3-Ⅱ、Beclin1蛋白表达水平升高,BNIP3蛋白表达水平降低(P<0.05)。结论 针刺和艾灸可能通过调控顺铂模型小鼠肝组织中LC3-Ⅱ、Beclin1及BNIP3的表达水平,增强肝细胞自噬保护性机制、抑制细胞凋亡,从而减轻因顺铂化疗所致的肝损伤,其中艾灸比针刺效果显著,这可能是针灸减轻顺铂所致肝损伤的作用机制之一。Objective To explore the effect and mechanism of acupuncture and moxibustion on liver injury induced by intraperitoneal injection of cisplatin from hepatocyte autophagy and apoptosis.Methods 40 SPF Kunming male mice were randomly divided into blank group,model group,acupuncture group and moxibustion group,10 mice in each group.Liver injury model was established by intraperitoneal injection of cisplatin.The mice in acupuncture group and moxibustion group were respectively treated at"Dazhui""Ganshu""Shenshu"and"Zusanli",once a day for 5 consecutive days.The pathological changes of liver tissue were observed by HE staining.The contents of ALT,AST and GLDH in serum of mice in each group were detected by ELISA.LC3-Ⅱ,Beclin1 and BNIP3 in liver tissue were detected by western blot.Results Compared with the blank group,the contents of ALT,AST and GLDH in serum and the expression of LC3-Ⅱ,Beclin1 and BNIP3 in liver tissue of model group increased(P<0.05).Compared with model group,the contents of ALT,AST and GLDH in serum decreased,while the expression of LC3-Ⅱand Beclin1 protein increased and the expression of BNIP3 protein decreased significantly in liver tissue of mice in acupuncture and moxibustion group(P<0.05).Compared with acupuncture group,the expression levels of LC3-Ⅱand Beclin1 protein in moxibustion group were higher,while the expression level of BNIP3 protein was lower in moxibustion group(P<0.05).Conclusion Acupuncture and moxibustion may enhance the protective mechanism of hepatocyte autophagy and inhibit apoptosis by regulating the expression of LC3-Ⅱ,Beclin1 and BNIP3 in the liver tissue of cisplatin model mice,thus alleviate the liver injury caused by cisplatin.Moxibustion is more effective than acupuncture.
