慢性持续性耳鸣门诊患者的临床特征分析

摘　　要：目的 了解门诊慢性持续性耳鸣门诊患者的临床特征。方法 对232例慢性持续性耳鸣门诊患者进行详细的病史采集、耳鸣调查问卷填写、耳鼻喉科专科检查、纯音听阈测试及耳鸣匹配检查。结果 1、男性41.81%(97/232),女性58.19%(135/232),女性明显高于男性,多在中年组;年龄主要集中在46~69岁组,达到63.36%(147/232);单耳多见,右耳多发;有62.93%(146/232)合并慢性疾病,其中以合并高血压居多;2、THI为2级的占32.76%(76/232),VAS 1~5分段者占62.5%(145/232),0.5~1年病程者占31.90%(74/232),按照病程分析THI和VAS评分发现,随着病程的延长,严重程度有所减弱;3、能成功耳鸣匹配的占63.36%(147/232),多为高频型,耳鸣的主调声频率主要集中在高频区73.47%,阈值多≥50dB为66.67%,响度多在0dB~5dB SL范围(72.79%),残余抑制实验总阳性达61.23%。结论 通过调查分析,初步反映了慢性持续性耳鸣门诊患者耳鸣发生持续的相关临床特征。Objective To report clinical characteristics of patients with chronic persistent tinnitus.Methods Data from patients with chronic persistent tinnitus(n=232)were collected including detailed medical history,tinnitus questionnaires,otolaryngology examination,pure tone thresholds and tinnitus characteristics.Results There were more female patients(135/232,58.19%)than male patients(77/232,41.81%),mostly 46~69 years of age(147/232,63.36%).Unilateral tinnitus in right ear was more common.Chronic systemic diseases were present in 146 patients(62.15%),especially hypertension.THI was graded as“mild”in 76 patients(32.58%),and 145 patients(62.50%)rated their tinnitus as 1~5 on VAS.In about 1/3 of the patients(74/232,31.90%),the course of tinnitus was 0.5~1 year.Severity of tinnitus,as measured by the THI and VAS scores,decreased as the course of disease was prolonged.Tinnitus matching was successful in 63.36%of the patients(147/232),with most matched to high-frequency ranges(78.62%)with the loudness mostly in the range of 0~5 dB SL(72.79%)and hearing thresholds at greater than 50 dB(66.67%).Residual inhibition was detected in 61.23%of the patients.Conclusion The current study provides a depiction of clinical characteristics of patients with chronic persistent tinnitus.
