老年突发性耳聋与颈动脉椎动脉超声表现相关性分析

摘　　要：目的 本文旨在探讨颈动脉椎动脉超声表现与老年突发性耳聋的程度、类型及预后的关系,为临床诊疗提供依据。方法 采用回顾性分析方法研究我科133例老年突发性耳聋患者临床资料,分析颈动脉、椎动脉超声多普勒结果,探讨其与老年突发性耳聋的严重程度、类型和预后结果的相关性。结果 颈动脉内膜厚度与老年突发性耳聋的听力损失程度和类型无相关性,椎动脉阻力指数与其听力损失程度及类型存在负相关性;在预后不良组中,颈动脉硬化的病例数量显著高于预后良好组。结论 椎动脉阻力指数与颈动脉硬化对老年突发性耳聋程度和预后有一定相关性,对其诊疗和病情评估具有较高价值。Objective The purpose of this study was to analyze the correlation between the ultrasonography manifestations and the severity and prognosis of older patients with idiopathic sudden sensorineural hearing loss(ISSNHL).Methods The clinical data of 133 older patients with ISSNHL were retrospectively analyzed.The ultrasonic doppler results of carotid artery and vertebral artery were analyzed to clarify the correlation between them and the severity and prognosis of older patients with ISSNHL.Results Correlation analysis showed that there was no correlation between carotid artery intima-media thickness and the degree and type of hearing loss of older patients with ISSNHL,but there was a negative correlation between vertebral artery resistance index and the degree and type of hearing loss.In the group with poor prognosis,the number of cases with carotid atherosclerosis was significantly higher than that in the group with good prognosis.Conclusions Vertebral artery resistance index and the degree of carotid atherosclerosis are of great value in the diagnosis and condition evaluation of sudden deafness in the elderly.
