摘　　要：目的 探讨胰岛素样生长因子-1(insulin-like growth factor-I,IGF-I)和细胞核增殖相关抗原Ki-67在中耳胆脂瘤上皮中的表达及意义。方法 30例中耳胆脂瘤组织标本为实验组,10例耳后正常皮肤组织为对照组,实验组依骨质破坏程度分为轻度组和重度组,采用免疫组化二步法检测中耳胆脂瘤上皮及正常皮肤表皮中IGF-I及Ki-67的表达,计算Ki-67增殖指数(proliferation index,PI)。结果 胆脂瘤组上皮中IGF-I的表达高于对照组(0.24±0.08 vs.0.06±0.04,t=7.26,P=0.000,P<0.001),重度组IGF-I的表达高于轻度组(0.29±0.05 vs.0.19±0.06,t=4.80,P=0.000,P<0.001)。胆脂瘤组上皮的PI高于对照组(19.6%±8.8%vs.5.6%±2.1%,t=8.04, P=0.000,P<0.001),重度组的PI高于轻度组(23.4%±6.6%vs.15.9%±9.4%,t=2.52,P=0.018,P<0.05)。胆脂瘤组上皮中的PI与同一标本相邻切片中IGF-I的表达密切相关(r=0.51,P=0.004,P<0.05)。结论 IGF-I在中耳胆脂瘤上皮中呈高表达,IGF-I与中耳胆脂瘤上皮细胞过度增殖及骨质破坏程度密切相关。Objective To report expression of insulin-like growth factor-1(IGF-I)and Ki-67 in cholesteatoma.Methods Specimens were collected from 30 patients with middle ear cholesteatoma treated at the First Affiliated Hospital of Jinan University from January 2005 to June 2019.Cases were graded as mild or severe according to preoperative CT and intraoperative findings regarding bone destruction.Specimens of normal skin(n=10)were obtained from the retroauricular area and used as controls.Immunohistochemistry was used to detect IGF-I and Ki-67 expression with proliferation index(PI)calculated for Ki-67.Results IGF-I expression was higher in cholesteatoma than in normal skin(0.24±0.08 vs.0.06±0.04,t=7.26,P=0.000,P<0.001),and higher in severe cases than in mild cases(0.29±0.05 vs.0.19±0.06,t=4.80,P=0.000,P<0.001).PI of Ki-67 was also higher cholesteatoma than in normal skin(19.6%±8.8%vs.5.6%±2.1%,t=8.04,P=0.000,P<0.001),and higher in severe cases than in mild cases(23.4%±6.6%vs.15.9%±9.4%,t=2.52,P=0.018,P<0.05),and closely correlated to the expression of IGF-I in adjacent sections of the same specimen(r=0.51,P=0.004,P<0.05).Conclusion IGF-I expression and PI of Ki-67 are increased in the epithelium of middle ear cholesteatoma and associated with disease extent.They also correlate with each other.
