三级医院妊娠相关病例住院费用的影响因素分析

摘　　要：目的分析广州市某区属三级医院妊娠相关病例住院费用构成,了解影响医疗费用的相关因素,为区域医院开展合理控制医疗费用的过快增长提供参考。方法收集区域内2013—2021年出院时妊娠有关病例的基本信息,采用Pearson双变量相关分析、U检验及多元逐步回归分析,对影响住院费用高低的因素进行统计分析。结果共纳入妊娠有关病例142730例,平均年龄29岁,平均住院天数约5天,年均住院费用5781.23元,且2017—2021年均高于年均费用,年均增长速度为4.87%;住院费用构成主要是手术费、卫生材料费、药品费等,其中,年龄、住院天数、手术费、卫生材料费、药品费等因素对住院费差异具有统计学意义(P<0.05)。结论从适宜育龄群体、手术适应症、住院天数、控制药耗费比例、区域医院专科建设等方面入手,建立科学的诊疗标准及完善临床路径是控制医疗费用增长的重要手段。Objective By analyzing the composition of hospitalization expenses of pregnancy related cases in a third level hospital in Guangzhou,we can understand the factors affecting medical expenses and provide reference for regional hospitals to reasonably control the excessive growth of medical expenses.Methods Basic information of pregnancy-related patients discharged from hospital in the region from 2013 to 2021 was collected.Pearson bivariate correlation analysis and multiple stepwise regression analysis were used to analyze the influencing factors of hospitalization costs.Results A total of 142730 pregnancy related cases were included in this study,with an average age of 29 years.The average length of stay was about 5 days.The average annual hospitalization cost was 5781.23 yuan,and the average annual increase rate was 4.87%from 2017 to 2021.The hospitalization expenses were mainly composed of operation expenses,health materials expenses,medicine expenses,etc.Age,hospitalization days,operation expenses,health materials expenses,medicine expenses and other factors had a significant impact on the hospitalization expenses(P<0.05).Conclusion It is an important means to control the increase of medical expenses by establishing scientific diagnosis and treatment standard and perfect clinical pathway from the aspects of suitable reproductive age group,surgical indications,hospitalization days,control of drug consumption ratio and specialty construction in regional hospitals.
