大隐静脉高位结扎联合点式剥脱术治疗大隐静脉曲张的应用效果观察

摘　　要：目的:探讨对大隐静脉曲张患者采取高位结扎与点式剥脱术联合治疗的效果。方法:选取我院2021年5月至2022年10月期间收治的94例大隐静脉曲张患者作为研究对象,根据住院号将患者分为对照组和观察组(n=47),对照组采用大隐静脉高位结扎联合传统剥脱术治疗,观察组实施大隐静脉高位结扎联合点式剥脱术治疗。经治疗3 d后,对比两组患者的临床疗效,采用视觉模拟评分法(Visual analogus scale,VAS)评估患者疼痛程度,应用静脉曲张临床严重程度评分表(Venous clinical severity score,VCSS)评估患者病情,应用酶联免疫吸附法检测白细胞介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)水平,应用全自动凝血分析仪检测凝血酶原时间(Prothrombin Time,PT)、凝血酶时间(Thrombin Time,TT)、活化部分凝血酶时间(Activated Partial Thromboplastin Time,APTT)。结果:观察组治疗有效率显著高于对照组(P<0.05);观察组VAS评分与VCSS评分均明显低于对照组(P<0.05);观察组IL-6、TNF-α水平均显著低于对照组(P<0.05);观察组APTT、PT、TT均长于对照组。结论:在大隐静脉高位结扎的基础上联合点式剥脱术治疗大隐静脉曲张,减轻应激反应与术后疼痛,对患者凝血功能的影响较小。Objective:To investigate the effects of high ligation combined with point exfoliation in the treatment of varicose great saphenous veins.Methods:A total of 94 patients with varicose great saphenous veins admitted to our hospital from May 2021 to October 2022 were selected as research objects.According to the hospitalization number,the patients were divided into a control group and an observation group(n=47).The control group was treated with high ligation of the great saphenous vein combined with traditional exfoliation,and the observation group was treated with high ligation of the great saphenous vein combined with point exfoliation.After 3 days of treatment,the clinical efficacy of the two groups was compared,the degree of pain by visual analog scale(VAS),and the venous clinical severity score(VCSS)were evaluated,and the levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were measured by enzyme-linked immunosorbent assay(ELISA).Also,thrombin time(PT),thrombin time(TT),and activated partial thromboplastin time(APTT)were evaluated using an automatic thrombin analyzer.Results:The effective rate of the observation group was significantly higher than that of the control group(P<0.05).The VAS score and VCSS score of the observation group were significantly lower than those of the control group(P<0.05).The levels of IL-6 and TNF-αin the observation group were significantly lower than those in the control group(P<0.05).APTT,PT,and TT in the observation group were longer than those in the control group.Conclusion:The combination of high ligation of great saphenous vein and point exfoliation in the treatment of great saphenous vein varicose can reduce stress response and postoperative pain,and has little effect on the coagulation function of patients.
